消髓化核汤治疗环形增强破裂型腰椎间盘突出症30例

Thirty Cases of Ring-reinforced Ruptured Lumbar Intervertebral Disc Herniation Treated with Xiaosui Huahe Decoction

目的:观察消髓化核汤治疗环形增强破裂型腰椎间盘突出症的临床疗效。方法:选取苏州市中医医院2014年1月至2016年6月行增强核磁共振成像(magnentic resonance imaging,MRI)的30例有环形增强破裂型腰椎间盘突出症患者,给予消髓化核汤治疗,同时配合肌肉锻炼,按照腰椎JOA评分改善率及MRI吸收率对患者转归进行分析评定。结果:30例患者有5例退出研究(1例患者治疗期间因跌倒至腰椎骨折退出研究,1例患者个人意愿转手术治疗,3例患者因个人原因未来本院影像复诊退出研究),退出率为16.67%。经非手术治疗后24例患者症状好转明显,1例患者症状无明显好转。治疗前与治疗后1个月、3个月、6个月各时间点JOA评分改善率比较,差异具有统计学意义(P<0.05),治疗后12个月与治疗后6个月比较,差异无统计意义(P>0.05)。结论:消髓化核汤治疗环形增强破裂型腰椎间盘突出症,可显著改善患者的腰椎状况。

Objective： To observe the clinical curative effect of Xiaosui Huahe Decoction on ring-reinforced ruptured lumbar disc herniation. Methods： Thirty patients with ring-reinforced ruptured lumbar disc herniation who were treated in Suzhou City Hospital of TCM between January 2014 and June 2016 were chosen as the research subjects,and treated with Xiaosui Huahe Decoction(XSHHD） combined with muscle exercise. The improvement rate of lumbar JOA score and the rate of MRI absorption were analyzed and evaluated. Results： Among 30 patients,5 were withdrawn from the study(one patient withdrew from the fall to the lumbar fracture,one patient withdrew due to individual willingness to turn the operation,and 3 patients withdrew due to failure to go to hospital for further consultation for personal reasons）. The withdrawal rate was 16. 67%. After non operative treatment,the symptoms of 24 patients improved significantly,and the symptoms of one patient did not improve significantly. The difference of JOA score was statistically significant(P〈0. 05） before the treatment and one month,3 months and 6 months after the treatment(P〈0. 05）. There were no statistically significant differences between 12 months after the treatment and 6 months after the treatment(P〉0. 05）. Conclusion： In terms of ring-reinforced ruptured lumbar disc herniation,XSHHD can significantly improve the patient＇s lumbar condition.
