T3、T4、TSH和HGB的联合检测在儿童甲状腺功能减退症临床诊断中的意义 被引量:5

The significance and effect of T3,T4,TSH and HGB in clinical diagnosis of hypothyroidism in children

摘要 目的:探讨三碘甲状腺原氨酸(T3)、甲状腺素(T4)、促甲状腺激素(thyrotropin,thyroid stimulating hormone,TSH)和血红蛋白(hemoglobin,HGB)联合检测在儿童甲状腺功能减退症(甲减)临床诊断中的作用和意义。方法:选取2015年5月至2017年5月本院门诊收治的60例儿童甲减患者为观察组,选取同期健康体检儿童30例为对照组。收集患者血清,检测血清中T3、T4、TSH和HGB水平变化并进行统计分析。结果:与对照组相比,观察组患者血清内FT3、FT4、T3和T4浓度有明显降低,血清内TSH浓度有明显上升,HGB浓度明显下降,差异具有统计学意义(P<0.05)。T4、TSH和HGB各项指标在观察组患者的单项检测阳性率明显低于各指标联合检测的阳性率,差异具有统计学意义(P<0.05)。结论:儿童甲减患者血清中T3、T4、TSH和HGB含量呈异常表达状态;T3、T4、TSH和HGB的联合检测在儿童甲减的诊断中具有重要的临床意义。 Objective: To investigate the clinical value of combined detection of triiodothyronine( T3),thyroid hormone( T4),thyroid stimulating hormone( TSH) and hemoglobin( HGB). Methods: From May 2015 to May2017,60 cases of hypothyroidism in our hospital were selected as the observation group,30 healthy children in the same period were selected as the control group. The patient sera were collected and serum levels of T3,T4,TSH and HGB were measured for statistical analysis. Results: Compared with the control group,the concentration of FT3,FT4,T3 and T4 in the serum of patients in the observation group were significantly decreased, the concentration of TSH in serum was increased and the concentration of HGB was decreased significantly( P 0. 05). The individual test positive rate of T4,TSH and HGB in the observation group was significantly lower than that of the combined detection of each index,the difference was statistically significant( P 0. 05). Conclusion:The levels of T3,T4,TSH and HGB in children with hypothyroidism are abnormally expressed. The combined detection of T3,T4,TSH and HGB has important clinical significance in the diagnosis of hypothyroidism in children.
