规范化疼痛护理干预对胃癌晚期患者癌痛及生活质量的影响

Effect of standardized pain nursing intervention on cancer pain and quality of life in advanced gastric cancer patients

目的探讨规范化癌痛护理干预对胃癌晚期癌痛患者数字疼痛强度量表(numerical rating scale,NRS)评分及生活质量的影响.方法选取浙江省新昌县人民医院收治的63例胃癌晚期癌痛患者,依照随机数字表法分组,对照组31例,观察组32例,对照组给予常规护理干预,观察组于常规护理基础上进行规范化癌痛护理干预,干预6 mo后观察比较两组遵医行为、NRS评分及疼痛控制满意度,并统计两组干预前后生活质量评分.结果观察组总依从率为93.75%高于对照组70.97%,差异有统计学意义(P<0.05);干预后两组NRS评分较干预前显著降低,且观察组NRS评分2.80±0.67小于对照组4.50±0.48,差异有统计学意义(P<0.05);观察组疼痛控制满意度为90.63%高于对照组70.97%,差异有统计学意义(P<0.05);观察组干预后心理功能、躯体功能、物质生活状态及社会功能各维度评分高于对照组,差异有统计学意义(P<0.05).结论规范化癌痛护理干预可减轻胃癌晚期癌痛患者疼痛程度,且干预后癌痛治疗依从性高、癌痛控制满意度高,有助于提高患者生活质量.

AIM To explore the effect of standardized pain nursing intervention on the numerical rating scale （NRS） score and quality of life in advanced gastric cancer patients with cancer pain.METHODS A total of 63 advanced gastric cancer patients with cancer pain were selected at the Xinchang People＇s Hospital, and they were randomly divided into either a control group （31 cases） or an observation group （32 cases）. The control group was given routine nursing intervention, and the observation group was given standardized pain nursing intervention on the basis of routine nursing. After 6 mo of intervention, patients＇ compliance, NRS score, and satisfaction with pain were compared between the two groups. The quality of life scores of the two groups were also compared.RESULTS The total compliance rate of the observation group was 93.75% （30/32）, which was higher than that of the control group [70.97% （22/31）, P 〈 0.05]. After intervention, the NRS scores of the two groups were significantly lower than those before intervention, and the NRS score of the observation group （2.80±0.67） was significantly less than that of the control group （4.50± 0.48） （P 〈 0.05）. The rate of satisfaction with pain control in the observation group was 90.63% （29/32）, which was higher than that in the control group [70.97% （22/31）, P 〈 0.05]. The scores of psychological function, physical function, material life state, social function, and other dimensions in the observation group were higher than those of the control group （P 〈 0.05）.CONCLUSION Standardized pain nursing intervention can reduce the pain level in advanced gastric cancer patients with cancer pain and improve patients＇ compliance, satisfaction with pain control, and quality of life.
