宣肺止咳汤联合异丙托溴铵治疗咳嗽变异性哮喘(风寒犯肺)随机平行对照研究

Randomized Controlled Study on the Combination of Xuanfei Zhike Decoction(宣肺止咳汤) and Ipratropium Bromide in the Treatment of Cough Variant Asthma(Fenghan Fanfei/风寒犯肺)

[目的]观察宣肺止咳汤联合异丙托溴铵治疗咳嗽变异性哮喘(风寒犯肺)疗效。[方法]使用随机平行对照方法,将70例门诊患者按就诊顺序号方法随机分为两组。对照组35例异丙托溴铵,2喷/次,2次/d;沙美特罗替卡松粉吸入剂,1吸/次,2次/d。治疗组35宣肺止咳汤(黄芩、前胡、百部、紫菀、地龙、桑白皮、炙麻黄各10g,桔梗、杏仁各9g,蝉蜕、僵蚕各6g,生甘草3g),水煎200mL,100mL/次,2次/d;异丙托溴铵、沙美特罗替卡松粉吸入剂治疗同对照组。连续治疗2周为1疗程。观测临床症状、呼气峰值流速(PEF)、FEV_1/FVC、不良反应。治疗1疗程(2周),判定疗效。[结果]治疗组临床治愈18例,有效16例,无效1例,总有效率97.14%;对照组临床治愈13例,有效16例,无效6例,总有效率82.86%;治疗组疗效优于对照组(P<0.05)。PEF、FEV_1/FVC两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]宣肺止咳汤联合异丙托溴铵治疗咳嗽变异性哮喘(风寒犯肺),疗效满意,无严重不良反应,值得推广。

[Objective] To observe the effect of Xuanfei Zhike decoction combined with ipratropium bromide in the treatment of cough variant asthma （Fenghan Subiao）. [Methods] 70 outpatients were randomly divided into two groups by random parallel control method. The control group of 35 cases of ipratropium bromide spray, 2/time, 2 times/d; salmeterol xinafoate and dluticasone propionate pwder for inhalation, 1 suction/ time, 2 times/d. The treatment group 35 Xuanfei Zhike decoction （Huangqin,Qianhu, Baibu, Ziwan, Dilong, Sangbaipi, Zhimahuang each 10g, Jiegeng, Xingren each 9g, Cantui, Jiangchan each 6g, Shenggacao 3g）, decoction 200mL, 100 ml/times, 2 times/d; ipratropium bromide, treatment with Salmeterol Xinafoate and Fluticasone Propionate Powder for Inhalation the control group. Continuous treatment for 2 weeks was the 1 course of treatment. The clinical symptoms, peak expiratory flow velocity （PEF）, FEV1/FVC, and adverse reactions were observed. The 1 course of treatment （2 weeks） was treated and the curative effect was determined. [Results] In the treatment group, 18 cases were cured clinically, 16 cases were effective, 1 cases were invalid, the total effective rate was 97.14%. In the control group, 13 cases were cured, 16 cases were effective, 6 cases were ineffective, the total effective rate was 82.86%, the curative effect of the treatment group was better than that of the control group （P〈0.05）. The two groups of PEF and FEV1/FVC were improved （P〈0.01）, and the improvement of the treatment group was better than that of the control group （P〈0.01）. [Conclusion] the effect of Xuanfei Zhike decoction combined with ipratropium bromide in the treatment of cough variant asthma is satisfactory and has no serious adverse reaction. It is worth popularizing.
