经靶动脉灌注银杏叶提取物治疗血栓闭塞性脉管炎的效果分析 被引量:7

摘要 目的探索分析经靶动脉灌注银杏叶提取物(FPGBE)治疗组织坏死期血栓闭塞性脉管炎(LEN-TAO)的有效性。方法回顾性分析2014年6月至2017年7月本中心收治的61例LEN-TAO进行FPGBE治疗的临床资料,评估该方案的可行性及有效性。结果手术成功率100%,围术期及随访6个月内无死亡及截肢并发症发生,随访61例,使用医用红外热像仪,患肢在术前后的疼痛冷感评分显著降低(P<0.01),红外线定点温度分析肢体溃疡中心及周围组织皮温显著增高(P<0.05),跛行距离随访期间显著延长(P<0.05),随访期踝肱指数(ABI)均优于术前(P<0.05)。结论经外周动脉对血栓闭塞性脉管炎(TAO)的药物灌注治疗方案可以显著的改善肢体炎性反应,增高皮温,促进组织愈合,改善肢体存活率,提高生活质量,该治疗方案操作简单有效。此外,长期的结果仍然需要随访研究。 Objective To investigate the efficacy of transcatheter arterial infusion of Ginkgo biloba extract(FPGBE) in the treatment of thromboangiitis obliterans(LEN-TAO). Methods The clinical data of 61 patients with LEN-TAO treated with FPGBE from June 2014 to July 2017 were retrospectively analyzed to evaluate the feasibility and effectiveness of this strategy. Results The success rate of operation was 100%. There was no amputation during the perioperative period and follow-up period of 6 months. The pain relief scores of 61 limbs were significantly reduced(P〈0.01), resting pain relief, infra-red fixed Point temperature analysis Limb skin temperature was significantly increased(P〈0.05), limp home distance was significantly longer(P〈0.05), the follow-up ankle-brachial index(ABI) were better than preoperative(P〈0.05). Conclusion Transcatheter arterial thrombectomy(TAO) by drug infusion therapy program can significantly improve the limb inflammatory response, increased skin temperature, promote tissue healing, improve limb survival and improve quality of life, the treatment is simple and effective. In addition, long-term outcomes still require follow-up studies.
