射频消融在血栓闭塞性脉管炎中的应用

目的探讨射频消融术在治疗血栓闭塞性脉管炎(TAO)中的应用。方法回顾分析本院自2016年4月至2017年7月期间对7例TAO患者常规采用腔内技术球囊扩张术的同时进行血管腔内射频消融术,通过观察患者血管通畅程度、疼痛消失情况、当晚睡眠情况、下肢皮温改善情况、踝肱指数(ABI)等评判临床疗效。结果 7例患者手术均获得成功,术后临床症状均得到改善,5例患者疼痛术后即刻消失,2例患者术中出现动脉痉挛当晚疼痛消失,当晚起睡眠得到改善,患肢皮温得到明显改善,ABI均提高0.5以上。患者在随访期间临床症状保持良好,均无复发,CTA显示动脉血流通畅。结论利用血管腔内射频消融技术去股浅动脉交感神经化治疗TAO,能够显著改善患者的机体状况,提升其临床治疗总有效率,值得在临床上推广应用。

Objective To discuss the application of radiofrequency ablation in the treatment of thromboangiitis obliterans（TAO）. Methods We are retrospectively analyzed in our hospital from April 2016 to July 2017 7 patients with TAO routine use intraluminal balloon dilatation of intracavitary radiofrequency ablation at the same time, through the patient’s vascular patency, the disappearance of pain, the evening sleep, lower limb skin temperature to improve the conditions and ankle brachial index（ABI） evaluation of clinical curative effect. Results 7 patients had successful operation. The clinical symptoms were improved after operation. 5 patients disappeared the pain after the operation, 2 patients who appeared during the operation of arterial spasm the pain disappeared on the night, the sleep improved from the same night, the skin temperature was obviously improvement and ABI increased by more than 0.5. During follow-up, the patient’s clinical symptoms remained well, with no recurrence. CTA showed a smooth arterial blood flow. Conclusion To treat TAO by sympathetic nerve radiofrequency ablationcan significantly improve the patient’s condition and improve the total effective rate of clinical treatment, is worth the clinical promotion.
