单、双侧脑室外置管引流治疗高血压性脑出血破入脑室的效果观察

Comparison of clinical effects of single and bilateral external ventricular drainage in the treatment of hypertensive intracerebral hemorrhage breaking into ventricles of brain

目的对比分析单侧与双侧脑室外置管引流治疗高血压性脑出血破入脑室的临床效果。方法选择收治的高血压性脑出血破入脑室患者94例,按照随机数字表法分为单侧组与双侧组,每组47例。单侧组患者实施单侧脑室外置管引流,双侧组患者实施双侧脑室外置管引流,比较2组患者血肿消除时间、引流治疗时间、引流通畅情况、住院时间,随访1年预后情况及并发症发生情况。结果双侧组患者血肿消除时间为(16.49±3.24)d,快于单侧组的(23.24±5.16)d;双侧组引流治疗时间为(8.57±1.54)d,快于单侧组的(11.49±1.98)d。双侧组患者引流通畅率为89.36%,高于单侧组的72.34%;双侧组住院时间为(18.41±2.78)d,短于单侧组的(24.32±4.10)d,差异均有统计学意义(P<0.05);双侧组预后良好率(40.43%)高于单侧则(23.40%,P<0.05),双侧组并发症发生率(8.51%)低于单侧组(25.53%,P<0.05)。结论双侧脑室外置管引流治疗高血压性脑出血破入脑室临床效果优于单侧脑室外置管引流。

Objective To compare and analyze the clinical effects of unilateral and bilateral ventricular external drainage in the treatment of hypertensive intracerebral hemorrhage breaking into ventricles. Methods 94 patients with hypertensive intraeerebral hemor- rhage admitted were divided into unilateral group and bilateral group, with 47 cases in each group. Unilateral group of patients with uni- lateral external ventricular drainage, bilateral group of patients with bilateral ventricular external drainage. The time of hematoma elimina- tion,the time of drainage treatment, the pateney of drainage and the length of stay were compared between the two groups. The prognosis and complications were observed after 1 years of follow - up. Results The bilateral group hematoma elimination time was （ 16.49 ± 3.24）d,which was less than the unilateral group （23.24±5.16） d. The drainage treatment time was （8.57 ± 1.54）d,which was less than the unilateral group （ 11.49±1.98 ） d. The drainage rate （ 89.36% ） was higher than the unilateral group （72.34%）. The hospi- talization time （18.41 ± 2.78 ）d was less than the unilateral group （24.32±4.10 ）d （P 〈 0.05 ）. The good prognosis rate （40.43%） was higher than the unilateral （23.40%） （ P 〈 0.05 ）. The complication rate of bilateral group （ 8.51% ） was lower than the unilateral group （ 25.53 % ） （P 〈 0.05 ）. Conclusion The treatment of hypertensive cerebral hemorrhage by double ventricle external drainage is better than unilateral lateral ventricle drainage.
