可切除胰腺癌新辅助治疗与直接手术的选择与争议 被引量：3

摘要 胰腺癌是高度恶性的消化系统肿瘤,起病隐匿,预后差,其肿瘤相关病死率位居第4位.30年来,随着外科手术技术的不断进步,尽管胰腺癌围手术期病死率及术后并发症发生率明显下降,胰腺癌患者的总体生存时间仍无大幅度提升.约80％的患者在确诊时已失去手术机会,而对于可切除胰腺癌,其术后5年生存率仅约为20％.随着胰腺癌治疗理念的转变,胰腺癌的新辅助治疗逐渐受到关注.美国国立综合癌症网络（National Comprehensive Cancer Network,NCCN）指南推荐所有可能切除的胰腺癌患者行新辅助治疗,而对于可切除胰腺癌是否应用新辅助治疗或直接手术尚存在争议.本文将结合可切除胰腺癌新辅助治疗的相关研究,总结可切除胰腺癌新辅助治疗的提出背景、适应证及临床意义,比较新辅助治疗与直接手术对胰腺癌患者预后的影响,并结合笔者的临床经验,讨论新辅助治疗与直接手术的选择与争议.
