构建男性正常及膀胱出口梗阻膀胱尿道三维动态数学模型

摘要 目的利用男性正常、膀胱出口梗阻的CT影像资料和尿动力学检查资料,通过软件建立正常、膀胱出口梗阻男性三维动态膀胱尿道数学模型。方法 13例成年男性,4例(年龄45~58岁,中位数50岁)没有膀胱尿道疾病被认为正常组病人,9例(年龄43~60岁,中位数48岁)诊断明确膀胱出口梗阻病人。临床资料包括CT影像资料和完整的尿动力学资料。利用软件扫描所有病例的CT影像资料并对其进行处理及分析,建立动态三维正常、膀胱出口梗阻男性膀胱尿道数学模型。结果正常组及出口梗阻组的膀胱的排空、逼尿肌收缩的速率、逼尿肌做功有显著性差异,两组尿流加速度之间没有发现显著性的差异。结论通过模型动态演示发现正常组与膀胱出口梗阻组病人在三维动态膀胱尿道模型表现方面的差异;且发现了在正常与膀胱出口梗阻男性之间逼尿肌收缩速率与膀胱排空之间的不一致性。 Objective Combined CT images and urodynamic results on normal and baldder outlet obstruction patients.Created 3 D dynamic male bladder and urethral digital mode. Methods 13 cases male including 4 cases who do not have bladder and urethral diseases as normal and 9 cases who were diagnosised bladder outlet obstruction. Clinical material included bladder and urethral CT images and data. Using the software analyse the data. Created 3 D bladder and urethral digital model. Results The emptying process of bladder、the velocity of contraction of detrusor and the detrusor work ave a significant difference between normal and baldder outlet obstruction patients. Non significant difference had been found in acceleration of urine flow between both. Conclusion By the 3 D dynamic model,We found that the performances of 3 D dynamic model and detrusor contraction were difference between the bladder outlet obstruction group and normal group.
