倒V形松解术在下睑内翻倒睫伴内眦赘皮矫正中的应用

Application of inverted“V”release surgery in the correction of epicanthus with entropion trichiasis in the lower eyelid

目的观察一种新的矫正方法(倒V形松解术)在先天性下睑内翻倒睫伴内眦赘皮中的应用效果。方法对先天性下睑内翻倒睫伴内眦赘皮的患者,采用倒V形松解术矫正内眦赘皮,赘皮切口线与下睑切口线相连续,双层缝合法矫正下睑内翻倒睫;分别于术后1个月和6个月,对患者的睑裂宽度、泪阜显露率、睫毛朝向、切口瘢痕情况进行评估。结果术后随访6~18个月。术后6个月,睑裂宽度、泪阜显露率、睫毛朝向与术前相比差异均具有统计学意义(P<0.05);85%内眦切口瘢痕在术后6个月时不可见。结论倒V形松解术矫正先天性下睑内翻倒睫伴内眦赘皮,可取得功能和美观的双重效果,值得临床推广应用。

Objective To observe the efficacy of a new correction method（inverted＂V＂release surgery） on congenital entropion trichiasis in the lower eyelid with epicanthus. Methods Totally 20 patients underwent inverted ＂V＂release surgery to correct the epicanthus. The incision of the epicanthus and entropion were continuous. Lower eyelid entropion trichiasis was corrected by two-layer suture.The width of the palpebral fissure, the exposed ratio of the lacrimal caruncle, eyelash direction, and incision scar were each assessed in the first and sixth month after surgery. Results All patients were followed up for 6 to 18 months. Half a year after surgery, the width of the palpebral fissure, the exposure ratio of the lacrimal caruncle, and the orientation of the eyelashes were statistically different from those before surgery（P〈0.05）. Eighty-five percent of the inner canthal incision scars were not visible at 6 months post-surgery. Conclusion From our experience, a doubly positive outcome of functionality and aesthetics can be obtained by using the inverted＂V＂release surgery method to correct congenital entropion trichiasis in the lower eyelid combined with epicanthus. This procedure should be given consideration for clinical application.
