摘要
肝移植已成为挽救终末期肝病患者唯一有效的手段。由于供体短缺,扩大标准供体(ECD)不得不被临床所应用。此类供肝在某种程度缓解了供体的短缺,但随之而来的是因供肝的缺血缺氧、脂肪变性等引起的一系列移植术后并发症。其中肝移植术后非吻合LI胆管狭窄(NAS)即是ECD肝移植后所带来的主要并发症之一。机械灌注(MP)对离体供肝保护作用的研究已不胜枚举。本文就移植术后NAS的发生机制以及MP降低NAS发生率等方面的研究做一综述。
Liver transplantation has become the most ef- fective treatment for end-stage liver diseases. Due to the shor- tage of organ, more and more extended criteria donors ( ECD ) grafts had been used, which expand the liver pool. However, a series of complications post transplantation were caused by ische- mia, bypoxia, steatosis and so on. The non-anastomotic biliary strictures after liver transplantation is one of the major complica- tions when the ECD donors was be used in clinic. The study on the protective effect of machine perfusion on liver donors is too numerous to list, and existing studies have found that MP can reduce the incidence of NAS after liver transplantation. This re- view provides an overview of the pathogenesis of NAS and the re- duction incidence of NAS by MP.
