药物和器械治疗后慢性心力衰竭患者峰值摄氧量预测预后节点的改变 被引量:7

摘要 峰值摄氧量(PVO_2)是评估可活动慢性心力衰竭(CHF)患者预后的有效的量化指标。既往PVO_2评估CHF患者预后的节点是β受体阻滞剂的广泛应用之前经临床验证的。药物和器械治疗等新近治疗方法明显提高CHF患者的生存率。本文就现代药物和器械治疗CHF后PVO_2等预测预后节点的变化情况进行综述,以满足临床的CHF患者预后评估和治疗策略选择的需要。多项临床数据表明,不能耐受β受体阻滞剂的CHF患者预测不良预后的节点PVO_2=14ml/(kg·min)不变;而对于服用β受体阻滞剂的CHF患者,PVO_2预测其不良预后的节点变为12 ml/(kg·min)。心脏再同步治疗(CRT)对PVO_2评估CHF患者预后的节点无影响。根据12 ml/(kg·min)<PVO_2<14ml/(kg·min)评估的中危分层CHF患者,需进一步使用心力衰竭生存评分(HFSS)模型进行评估。对于青少年CHF患者,峰值摄氧量预计值百分比(%p PVO_2)可作为补充评估措施。
