超微血管成像技术和数字减影血管造影技术在诊断颈内动脉重度狭窄及闭塞的对比研究初探 被引量:9

摘要 目的:探讨超微血管成像技术(SMI)在颈内动脉重度狭窄近闭塞中的应用价值。方法:选取常规彩色多普勒超声检查诊断为颈内动脉起始段重度狭窄及闭塞患者32例,共34条动脉。数字减影血管造影(DSA)检查结果作为金标准,将SMI、DSA诊断颈内动脉狭窄程度结果进行对比研究。结果:SMI与DSA诊断颈内动脉重度狭窄及闭塞的一致性Kappa值为0.735,提示SMI技术诊断颈内动脉重度狭窄及闭塞结果可靠。结论:SMI技术操作方便、无创、无放射性,可以判断颈内动脉起始段管腔狭窄程度,具有较高的准确度及灵敏度,可以作为诊断颈内动脉狭窄的首选影像学筛查手段。 Objective: Investigate the application value of superb micro vascular imaging(SMI) in severe stenosis near occlusion of internal carotid artery(ICA). Methods: Selecting 32 patients(34 vessels) with severe ICA stenosis and occlusion diagnosed by conventional color Doppler ultrasound. The result of ICA stenosis with SMI were compared with digital Subtraction Angiography(DSA) result used as the gold standard. Results: The Kappa value of SMI and DSA in the diagnosis of severe internal carotid stenosis and occlusion is 0. 735,indicating SMI is reliable in the diagnosis of severe stenosis of ICA. Conclusion: SMI is easy to operate,noninvasive and radioactive. It can be used to judge the degree of stenosis of ICA with high accuracy and sensitivity. It can be used as the first imaging screening method in the diagnosis of ICA stenosis
