腹腔镜阑尾切除Hem-o-lok夹闭与荷包缝合包埋残端的处理效果比较

Comparison on the effects of Hem-o-lok clipping and purse string suture for in the laparoscopic appendectomy

目的比较腹腔镜阑尾切除Hem-o-lok夹闭与荷包缝合包埋残端的处理效果。方法按照随机数字表法将116例择期行腹腔镜阑尾切除术的患者均分为观察组和对照组各58例,观察组患者采取Hem-o-lok夹闭处理阑尾残端,对照组患者采取荷包缝合包埋处理残端,比较两组患者手术效果。结果观察组手术总时间显著低于对照组,治疗总费用及阑尾根部处理材料费均显著高于对照组,差异有统计学意义(P<0.05);两组患者术后住院时间和并发症总发生率对比,差异无统计学意义(P>0.05)。结论腹腔镜阑尾切除Hem-o-lok夹闭与荷包缝合包埋处理阑尾残端均安全有效,Hem-o-lok夹闭操作简单,但荷包缝合包埋手术费用相对便宜,可根据实际情况向患者推荐。

Objective To compare the effect of Hem-o-lok clipping and purse string suture in the laparoscopic appendectomy. Methods According to the random number table method,116 patients underwent laparoscopic appendectomy were randomly divided into observation group and control group with 58 cases in each group. Patients in the observation group were taken Hem-o-lok clipping for treating appendiceal stump,the control group was treated by suture embedding for the treatment of stump,the effect of the operation were compared.Results In the observation group,the total operation time was significantly lower than the control group,however,the total cost of treatment and the cost of materials in the treatment of appendix were significantly higher than control group,and the differences were statistically significant（ P〈0.05）. There was no statistically significant difference between the two groups in hospitalization time and occurrence of complications after operation（ P〉0.05）.Conclusions Both Hem-o-lok clipping and suture embedding in the treatment of appendiceal stump are safe and effective in laparoscopic appendectomy. Hem-o-lok clipping operation is simple,but the suture embedding operation is relatively inexpensive,so the operation method can be recommended to the patients according to the actual situation.
