正畸微种植体植入早期牙周组织凋亡相关蛋白Bcl-2和Bax的表达及意义

Early expression and significance of Bcl-2 and Bax in orthodontic micro-implant

目的探讨微种植体植入早期牙周组织B淋巴细胞瘤-2基因(Bcl-2基因)和Bcl-2相关x蛋白(Bax)的表达与意义。方法以36例预行微种植体治疗的安氏Ⅱ类1分类错牙合畸形患者为研究对象,分别于矫治前后1周取种植体周围组织,提取细胞体外培养72 h,获得细胞以WB实验测定Bcl-2与Bax表达,上层培养液以ELISA实验测定白细胞介素-1β(IL-1β)、IL-8、单核细胞趋化蛋白-1(MCP-1)水平。结果植入1周时,Bcl-2蛋白表达水平和Bcl-2/Bax明显降低,Bax表达水平升高,同时IL-1β、IL-8、MCP-1水平明显升高,与治疗前相比,差异有统计学意义(P<0.05);Bcl-2/Bax与IL-1β、IL-8、MCP-1水平呈负相关(r=-0.888,r=-0.881,r=-0.859,P<0.001)。本组共发生9例并发症,并发症组植入后1周Bcl-2表达水平和Bcl-2/Bax低于非并发症组,Bax表达水平高于非并发症组,差异有统计学意义(P<0.05)。结论正畸微种植体植入早期Bcl-2/Bax水平与牙周组织排异炎性反应密切相关,对植入效果有一定的预测价值。

Objective To explore the early expression and significance of Bcl-2 and Bax in orthodontic micro-implant.Methods Totally 36 patients with Ann Ⅱ class 1 classification malocclusion deformity were selected as objects,whose periodontium was respectively extracted after treatment 1 week before and later,and the cells were extracted and cultured. After72 h,the Bcl-2 and Bax expressions of cells were detected by WB assay; the interleukin 1 beta（ IL-1β）,interleukin 8（ IL-8） and monocyte chemoattractant protein（ MCP-1） levels of culture solution were detected by enzyme-linked immuno sorbent assay（ ELISA） kits. Results At 1 week after implant,the levels of Bcl-2 and Bcl-2/Bax decreased significantly,the Bax expression level increased,the levels of IL-1β,IL-8,MCP-1 significantly increased,and compared with that before the treatment,the differences had statistical significance（ P〈0. 05）. The Bcl-2/Bax level was negatively correlated with IL-1β,IL-8 and MCP-1 at 1 week after implant（ r =-0. 888,r =-0. 881,r =-0. 859,P〈0. 001）. There were 9 cases of complications（ 25. 0%）,and 1 week after implant,the levels of Bcl-2 and Bcl-2/Bax of the patients with complications were lower than those without complications; the Bax expression was higher than those without complications,and the differences had statistical significance（ P〈0. 05）. Conclusion The early level of bcl-2/Bax after orthodontic micro-implants were closely related with rejection related inflammatory,and had some predictive value for the implant effect.
