中西医结合治疗肺癌合并恶性胸腔积液

Treatment of lung cancer complicated with malignant pleural effusion by combination of traditional Chinese medicine and western medicine

目的探讨龙葵葶苈汤内服联合顺铂胸腔内灌注化疗对合并大量胸水肺癌患者免疫指标的影响。方法选取我院就诊的大量胸水肺癌患者66例,将所有患者随机分为观察组和对照组,每组33例。观察组患者接受口服龙葵葶苈汤联合顺铂胸腔内灌注化疗,对照组患者单纯接受顺铂胸腔内灌注化疗进行治疗。疗程均为3周,对比分析2组患者治疗后的临床疗效、细胞免疫功能及不良反应发生状况。结果治疗后观察组患者CD3、CD4、CD8阳性T细胞百分比比治疗前明显升高(P<0.001),各项指标明显高于对照组(P<0.05);观察组客观有效率84.85%,明显高于对照组的42.42%,差异有统计学意义(P=0.001);观察组不良反应发生率12.12%,低于对照组的48.48%,但差异比较无统计学意义(P=0.338)。结论龙葵葶苈汤内服联合顺铂胸腔内灌注化疗治疗合并大量胸腔积液肺癌,治疗后机体细胞免疫功能增强,化疗不良反应的发生率降低。

Objective To explore the influence of Longkui Tingli decoction combined with cisplatin intrathoracic perfusion chemotherapy on cellular immune function of lung cancer patients with malignant pleural effusion. Methods Selected 66 cases of lung cancer patients with malignant pleural effusion treated in our hospital from August 2015 to July 2016. All the patients were randomly divided into the observation group and the control group, 33 cases in each group. The patients in the observation group were treated with Longkui Tingli decoction combined with cisplatin intrathoracic perfusion chemotherapy, the control group only received cisplatin Intrapleural Perfusion Chemotherapy, treatment for 3 weeks, compared the clinical curative effect, cellular immune function of two groups of patients after treatment and the adverse reaction incidence condition. Results After treatment, CD3, CD4, CD8 positive percentage of T cells of the observation group were increased significantly than before treatment（P 〈0.001） after treatment, indicators of the observation group were significantly higher than the control group（P 〈0.05）. The objective effective rate of the observation group was 84.85%, which was significantly higher than that of the control group（42.42%）（P = 0.001）. The incidence of adverse reactions in the observation group was 12.12%, which was lower than that in the control group（48.48%）, but the difference was not statistically significant（P = 0.338）. Conclusion The clinical efficiency of Longkui Tingli decoction combined with cisplatin intrathoracic perfusion chemotherapy on cellular immune function of lung cancer patients with malignant pleural effusion was higher, the cellular immune function was increased after treatment, and the incidence of chemotherapy adverse reaction was decreased.
