520例住院患者注射用脑蛋白水解物应用合理性分析

Analysis on Application Rationality of Cerebrolysin for Injections in 520 Inpatients

目的:了解住院患者注射用脑蛋白水解物的使用情况及其合理性,为临床合理用药提供参考。方法:随机抽取2016年6月至2017年6月江门市中心医院(以下简称"我院")使用注射用脑蛋白水解物的520例出院患者的病历资料,对注射用脑蛋白水解物的适应证、用法与用量及疗程等进行统计分析,评估用药合理性。结果:520例使用注射用脑蛋白水解物的患者中,男性271例,女性249例,年龄5~90岁,平均60.83岁;多来自于神经内科(207例,占39.81%),其次为神经外科、中医科等;用药时间多为1~7 d(346例,占66.54%),其次为8~14 d(158例,占30.38%);联合应用其他脑保护药的有114例(占21.92%);不合理使用注射用脑蛋白水解物共870例次,主要表现为用药疗程不适宜(362例次)、溶剂选择错误(191例次)、超适应证用药(145例次)及存在配伍禁忌(135例次)等。结论:我院住院患者注射用脑蛋白水解物的使用存在一定的不合理现象,需进一步规范用药行为,保证临床用药安全、有效。

OBJECTIVE： To investigate the application status and rationality of cerebrolysin for injections in inpatients,so as to provide reference for the rational drug application in clinic. METHODS： Medical records of520 inpatients with cerebrolysin in Jiangmen Central Hospital（ hereinafter referred to as ＂our hospital＂） from Jun.2016 to Jun. 2017 were randomly selected. Statistical analysis was conducted on indications,usage,dosage and treatment course of cerebrolysin for injections,and medication of rationality was evaluated. RESULTS： Of the520 patients with cerebrolysin for injections,males were 217 cases and females were 249 cases,and patients were aged from 5 to 90 years old,with the average of 60. 83 years old. Meanwhile,patients from department of neurology took the lead（ 207 cases, 39. 81%）, followed by department of neurosurgery and traditional Chinese medicine. The administration time were 1-7 d（ 346 cases,66. 54%）,followed by 8-14 d（ 158 cases,30. 38%）. There were114 cases（ 21. 92%） had drug combination with other cerebral protection drugs. There were 870 cases of irrational application of cerebrolysin for injections,mainly performed as improper treatment course（ 362 cases）,incorrect solvent selection（ 191 cases）,over indications（ 145 cases） and incompatibility（ 135 cases）,etc. CONCLUSIONS： There are still some irrational phenomenon in the application of cerebrolysin for injections in inpatients in our hospital. It is necessary to further standardize the drug behavior and ensure the safe and effective clinical medication.
