摘要
目的:探讨男性雄激素性秃发(AGA)患者毛囊中5α-还原酶的同工酶mRNA的表达及其临床意义。方法:研究对象为63例男性AGA患者和30例健康对照者。提取毛囊中的总RNA,经反转录后采用荧光定量聚合酶链反应(PCR)检测5α-还原酶的同工酶[Ⅰ型5α-还原酶(SRD5A1);Ⅱ型5α-还原酶(SRD5A2);Ⅲ型5α-还原酶(SRD5A3)]的基因表达。对AGA患者口服非那雄胺(保法止)连续治疗6个月,然后判断疗效。结果:拔发和毛囊单位提取术(FUE)两种方式获得的标本中都检测到3种5α-还原酶的表达,并且两种方法获取的标本检测结果差异无统计学意义(P>0.05)。SRD5A3在毛囊中的表达高于SRD5A1和SRD5A2的表达,3种5α-还原酶之间均呈正相关(P<0.05)。结论:可以通过拔发获取标本检测毛囊中的5α-还原酶mRNA表达。毛囊的内外毛根鞘中存在SRD5A3,其表达量与SRD5A1和SRD5A2的表达量相关,并且高于SRD5A1和SRD5A2的表达。
Objective: To investigate the expression and clinical significance of 5α-reductase mRNA in the hair follicles of male androgenetic alopecia. Methods: Total RNA from hair follicles of 63 male androgenetic alopecia patients and 30 healthy controls was extracted, followed by reverse transcription. Expression levels of 5α-reduetase isoform mRNA I-type 5α-redueed (SRDSA1), Ⅲ-type 5α-reductase(SRDSA2), Ⅲ-type 5α-reductase(SRDSA3)] were assessed by fluorescence quantitative polymerase chain reaction(PCR). Therapeutic efficacy was evaluated following treatments with finasteride(1 mg daily) for 6 months. Results: The expression levels of these three types of 5α-reduetase mRNA were comparable in hair follicles obtained by pulling hair out and by follicular unit extraction (FUE)(P〉0.05). Expression levels of SRDSA3 mRNA were higher than that of SRDSA1 and SRDSA2 mRNA. Expression levels of these three mRNA correlated positively with each other(P〈0.05). Conclusions: Expression of 5α-reduetase mRNA can be detected in hair follicles obtained by pulling hair out. SRDSA3 is present in the inner and outer root sheaths of hair follicles, and its expression levels are higher than that of SRD5A1 and SRD5A2. Expression levels of these three SRD5A mRNA correlate positively.
