胫骨中下段骨折远端内侧解剖钢板与外侧解剖钢板固定疗效比较

Comparison of therapeutic effect of Internal anatomic plate and lateral anatomic plate in treatment of middle and distal tibial fractures

目的：探讨胫骨远端内侧解剖钢板固定（使用MIPPO技术）治疗胫骨中下段骨折的疗效。方法：对我院于2013年7月～2015年7月收治的84例胫骨中下段骨折患者行胫骨远端内侧解剖钢板内固定和外侧解剖钢板内固定进行回顾性分析，主要观察了2组患者的手术指标，术后并发症及根据Kofeod评分标准对2组患者术后踝关节功能恢复评估。结果：观察组患者的内固定时间、术中出血量、骨折愈合时间、完全负重时间均小于对照组患者的时间，数据差异明显（P〈0．05）。对照组患者并发症的发病率明显高于观察组，数据差异明显（P〈0．05）。根据Kofeod评分标准对2组患者术后踝关节评分，观察组的优良率高于对照组的优良率，2组数据差异明显（P〈0．05）。结论：胫骨远端内侧解剖钢板固定治疗胫骨中下段骨折，能够明显提高骨折愈合的速度，加强恢复踝关节活动功能，是比较理想的治疗方式。

Objectiv： To investigate the effect of medial Internal anatomic plate fixation in the treatment of distal tibial fractures.Methods： the effect of 84 cases of middle and distal tibial fractures treated with LISS plate fixation and lateral anatomic plate fixation were retrospectively analyzed in our hospital in July 2013 and 2015 July. The paper observed the operation index of the two groups of patients, postoperative complications and with postoperative ankle restoration according to Kofeod score of two groups of patients. Results： The time of internal fixation, the amount of bleeding, the time of fracture healing, and the time of full weight bearing of the observation group were less than those of the control group and the difference was significant （ P 〈 0.05 ）. The incidence of complications in the control group was significantly higher than that in the observation group, and the difference was significant （ P 〈 0.05 ）. According to the Kofeod score standard for the two groups of patients with postoperative ankle score, the observation group＇s excellent rate was higher than the control group＇s excellent rate and the difference between the two groups was significant （P 〈 0. 05）. Conclusion：Internal anatomic plate fixation in the treatment of distal tibial fractures could significantly improve the rate of fracture healing, and enhance the recovery of ankle function. It might be an ideal treatment method.
