农村老年高血压人群抑郁症状的发生水平及影响因素

Prevalence and Influencing Factors of Depression Among Elders with Hypertension in Rural Areas

目的了解农村老年高血压人群抑郁症状的发生水平,分析其影响因素。方法对东莞市企石镇农村老年高血压人群进行调查,收集他们的抑郁症状、人口统计学资料、社会支持、合并慢性病情况等。结果共调查农村老年高血压患者846人,合并抑郁症状的比例为37.7%(319/846),单因素分析显示,无抑郁组与抑郁组在主观支持、社会支持利用度、居住情况和是否合并其他慢性病等方面差异有统计学意义。多元logistic回归分析显示,主观支持低分组、社会支持利用度低分组和合并慢性病的老年高血压患者患抑郁风险增高,OR值分别是1.831(1.239~2.504)、3.149(2.032~4.491)和2.426(1.683~3.179)。结论农村老年高血压人群容易合并抑郁,应提高该人群的社会支持度,加强慢性病治疗。

Objective To estimate the prevalence of depressive symptoms among elderly with hypertension; to determine influencing factors of depressive symptoms. Methods Survey sites were primary care settings in Qishi,Dongguan,Guangdong Province.Depressive symptoms,demographic data,combined chronic disease,and social support among elders with hypertension were investigated. Results The incidence of depressive symptoms was 37. 7%（ 319/846） among elderly with hypertension in rural areas. Univariate analysis showed that subjective support score,social support utilization score and major chronic disease were different between non depression group and depression group. Logistic regression analysis showed that low subjective support score（ OR = 1. 831,95%CI： 1. 239 ~ 2. 504）,low social support utilization score（ OR = 3. 149,95% CI： 2. 032 ~ 4. 491） and major chronic disease（ OR =2. 426,95% CI： 1. 683 ~ 3. 179） were risk factors of depression symptoms among elderly with hypertension. Conclusion Elderly hypertensive patients often merge with depression,the social support and treatment chronic diseases should be improved.
