营养评估方法在老年维持性血液透析患者营养评估中的作用评估

Effect of nutritional assessment method on nutritional assessment of elderly maintenance hemodialysis patients

目的:探究营养评估方法对于老年维持性血液透析患者的营养状态的评估效果。方法:抽取2016年7月~2017年7月在我院就诊的老年维持性血液透析患者23例作为观察组,另选择同期就诊于我院的健康老年人23例为对照组,分别应用主观整体营养评估(SGA)和营养不良-炎症评分(MIS)进行营养状态的评估,测定受试人员生化指标并与营养状态评估结果计算相关性。结果:观察组Scr、BUN、SGA评分和MIS评分与对照组有显著差异(P<0.05),其余指标无显著差异;相关性分析:观察组SGA的评分与TSF相关,MIS评分与体重、Alb相关(P<0.05)。对照组中,SGA的评分与BMI和TSF相关,MIS评分则与TSF、Scr、Hb和BUN水平相关。多元线性回归分析显示,对照组SGA、MIS两项评分与营养指标的复相关系数相近(P<0.05),而观察组中MIS的评分与营养指标成复相关(P<0.05)。结论:老年维持性血液透析患者炎症状态较为明显,MIS评分更适用于此类患者的营养状态评估。

Objective： To evaluate the effect of nutritional assessment in elderly maintenance hemodialysis patients. Methods： 23 elderly maintenance hemodialysis patients from July 2016 to July 2017 were selected as observation group and 23 healthy elderly patients in our hospital as control group. The nutritional status was evaluated by subjective global nutrition assessment （SGA） and malnutrition / inflammation score （MIS）, and the biochemical indexes of the subjects were measured and the correlation was calculated with the results of nutritional status assessment. Results： There was significant difference in Scr-BUNGA-score and MIS score between the observation group and the control group （P〈0.05）, but there was no significant difference in the other indexes, and correlation analysis： The SGA score was correlated with TSF in the observation group,The correlation between MIS score and body weight／Alb was significant （P〈0.05）.The scores of SGA in the control group were correlated with BMI and TSF, the MIS score was correlated with the levels of liB and BUN. Multiple linear regression analysis showed that the multiple correlation coefficient between the two scores of SGA and MIS and nutritional index in control group was similar （P〈0.05）, but that between MIS score and nutritional index in obser- vation group was significantly higher than that in control group （P〈0.05）.Conclusion： the inflammatory state of elderly patients with maintenance hemodialysis is more obvious and the MIS score is more suitable for the evaluation of nutritional status of the elderly maintenance hemodialysis patients.
