血浆雌激素及雌激素受体α与男性不育患者精液质量的关系

Relationship between estradiol or estrogen receptor α in serum and semen quality of male infertile patients

目的:探讨不同精液质量的男性患者血浆中雌激素及雌激素受体α浓度与精子浓度,活动力和形态的关系。方法:对634名男性进行精液检查,按照精液的质量参数将受试对象分成5组:正常精子组(n=147),少精子症组(n=96),弱精子症组(n=159),畸形精子症组(n=192)和非梗阻性无精子症组(n=40)。采用ELISA测定各组血浆雌二醇(E2),雌激素受体α(ESRα)的浓度,比较组间差异并进行相关性分析。结果:精液正常组的血浆E2和ESRα浓度显著低于其他4组(P<0.05),其他4组间差异无统计学为意义,血浆E2,ESRα与精子浓度,前向运动精子百分比及正常精子形态百分比呈负相关;精子浓度,血浆E2和ESRα浓度在3个年龄组间差异具有统计学意义(P<0.05);年龄与血浆E2(r=0.114,P=0.006)和ESRα浓度(r=0.190,P<0.001)存在正相关关系。结论:血浆E,ESRα的浓度能够反映精子的质量,在男性不育的病因分析中具有一定的指导价值。

Objective： To estimate the relationship between estradiol or estrogen receptor α in serum and semen quality of male infertile patients. Methods： Atotal of 634 men were divided to 5 groups after semen examination according to the semen quality： normozoospermiagroup（ n = 147）,oligozoospermia group（ n = 96）,asthenozoospermiagroup（ n = 159）,teratospermia group（ n = 192）,and non-obstructive azoospermia group（ n = 40）. Levels ofestradiol（ E2） and estrogen receptor α（ ESRα） were detected byenzyme linked immunosorbent assay. The innergroup variances were compared,meanwhile the correlations were analyzed. Results： The level of E2 and ESRαin serumof normozoospermia group was significantly lower than thoseof oligozoospermia,asthenozoospermia,teratospermia and non-obstructive azoospermia group（ P〈0. 05）,while differences among the other 4 groups were not significant. The levels of E2 and ESRαin serum were negatively correlated with sperm concentration,forward motion or normal sperm morphology rate; The concentration of sperm,E2 and ESRα among the three age groupswere significantly different（ P〈0. 05）. Male age was positive correlated with the serumlevels of E2（ r = 0. 114,P = 0. 006）and ESRα（ r = 0. 190,P〈0. 001）. Conclusions： The E2 and ESRα level in serum can reflect semen quality,which mayprovide reference and potential value in the diagnosis of male infertility.
