3D腹腔镜与2D腹腔镜手术治疗子宫肌瘤的疗效比较

The Comparison of Therapeutic Effects between 3D and 2D Laparoscopy for Uterine Myoma Resection

目的比较3D腹腔镜与2D腹腔镜手术治疗因子宫肌瘤行腹腔镜全子宫切除术的临床疗效。方法选择2015年1月-2016年12月在广州市第一人民医院妇产科收治的70例子宫肌瘤拟行腹腔镜全子宫切除术的患者,随机分为3D腹腔镜组和2D腹腔镜组,每组35例。对2组患者的手术时间、出血量、阴道残端缝合时间、术后首次肛门排气时间、术后并发症及住院时间进行比较。结果 3D腹腔镜组手术时间、阴道残端缝合时间均显著短于2D腹腔镜组(P<0.05),3D腹腔镜组手术出血量明显少于2D腹腔镜组(P<0.05)。结论 3D腹腔镜较传统的2D腹腔镜操作更精确,解剖层次更清晰,降低手术难度,适于临床推广。

Objective To make a comparison of the therapeutic effect of 3D and 2D laparoscopy for uterine myoma resection.Methods 70 cases accepted laparoscopic resection from Jan.2015 to Dec.2016 were divided into Group A（ 2D laparoscopic resection） and Group B（ 3D laparoscopic resection）,35 cases in each group.The relevant indexes,including operation duration,blood loss,duration of semi-pouch suture of vaginal stump,the postoperative exhausting time,postoperative complications,and hospital day were analyzed.Results The operation duration and duration of semi-pouch suture of vaginal stump in Group B were significantly shorter than those in Group A（ P〈 0.05）,and the blood loss was less（ P〈 0.05）. Conclusions Compared with the 2D laparoscopy,the 3D laparoscopy is more accurate in operation,clearer in anatomical level and lower in the difficulty of surgery.It is suitable for clinical promotion.
