核素纯固体试餐胃排空显像对胃排空功能的研究

Study on Gastric Emptying Function by Gastric Emptying Imaging of Nuclide Pure Solid Test Meal

目的应用放射性核素标记的纯固体试餐行胃排空显像,对前、后位胃区动态影像进行比较,以期为临床提供纯固体试餐胃排空显像中胃排空功能的正常及异常定量参考指标。方法受试者口服^(99m)Tc-DTPA鸡蛋纯固体餐后,即刻应用双探头单光子发射计算机断层成像仪(SPECT)行胃区平卧前、后位连续动态采集120 min,采集结束后,勾划胃区感兴趣区(ROI),获得时间-放射性曲线,分别计算胃区平卧前、后位胃半排时间(GET 1/2)和2 h胃排空率。结果 30例胃肠功能正常者,^(99m)Tc-DTPA鸡蛋固体餐胃排空显像显示胃前、后位GET 1/2分别为(92.97±15.87)min、(68.60±8.24)min,前、后位胃GET 1/2比较,其差异有统计学意义(P<0.05);前、后位2 h胃排空率分别为(50.13±3.93)%、(71.57±9.84)%,前、后位2 h胃排空率比较,其差异有统计学意义(P<0.05)。前、后位GET1/2及2 h胃排空率的95%置信区间分别为(92.97±15.87)min,(68.60±3.08)min,(50.13±3.93)%,(71.57±3.68)%。结论^(99m)Tc-DTPA纯固体试餐胃排空检查是一种简便、安全、无创、及时且可重复的胃排空功能检查方法,值得在临床上推广使用。

Objective The radionuclide-labeled pure solid test meal was used for gastric emptying imaging to compare the dynamic images of the anterior and posterior gastric regions in order to provide normal and abnormal quantitative references of gastric emptying function in gastric emptying imaging of pure solid test meal to the clinic.Methods After oral administration of （99m）Tc-DTPA pure solid meal of eggs,the subjects were immediately applied a dual-probe single-photon emission computerized tomography（ SPECT） device to continuously and dynamically capture data of anterior and posterior supine gastric regions for 120 minutes. After the end of the collection,the area of interest（ ROI） in the gastric regions was delineated,and the time-radioactivity curve was obtained. The GET 1/2 and 2 h gastric emptying rate for anterior and posterior supine gastric regions were calculated. Results In 30 patients with normal gastrointestinal function,（99m）Tc-DTPA solid meal gastric emptying imaging showed that the GET 1/2 of the anterior and posterior gastric regions were（ 92.97± 15. 87） min and（ 68. 60 ± 8. 24） min,respectively. The difference was statistically significant（ P0.05）.The 2 h gastric emptying rate was（ 50.13±3.93） % and（ 71.57±9.84）%,respectively. The difference was statistically significant（ P〈 0. 05）. The 95% confidence intervals of GET1/2 and 2 h gastric emptying rate of the anterior and posterior gastric regions were（ 92.97 ± 15.87）min,（ 68.60±3.08） min,（ 50.13±3.93） %,and（ 71.57±3. 68） %,respectively.Conclusions The examination of （99m）Tc-DTPA pure solid test meal gastric emptying is a simple,safe,non-invasive,timely,and repeatable method for gastric emptying.It is worth promoting in the clinic.
