冠心病与合并肺部感染患者E-选择素第二及四外显子基因型分布比较 被引量:6

Distribution of genotypes of E-selectin second and fourth exons in coronary heart disease patients complicated with pulmonary infections:a comparative study

摘要 目的比较冠心病患者及冠心病合并肺部感染患者E-选择素第二、四外显子基因型分布及血清炎性因子水平的差异。方法选取2015年1月-2017年3月于医院诊治的冠心病合并肺部感染患者342例为研究组,同期选择302例冠心病无并发感染患者为对照组,采用聚合酶链反应-限制性片段长度多态性方法(PCR-RFLP)检测两组患者E-选择素第二外显子+G98T和第四外显子+A561C的基因多态性,同时以双抗体夹心法检测患者的血清炎性因子包括白细胞介素6、8(IL-6、IL-8)、C-反应蛋白(CRP)、降钙素原(PCT)和E-选择素水平的变化。结果两组患者的+G98T和+A561C的基因型比较差异均有统计学意义(P<0.05);研究组GG与GT、AA与AC基因型分布高于对照组(P<0.05);研究组患者血清E-selectin、IL-6、IL-8、CRP及PCT水平均高于对照组,差异有统计学意义(P<0.05)。结论冠心病合并肺部感染患者与无感染患者E-选择素第二、四外显子基因型分布差异有统计学意义,冠心病合并肺部感染患者血清学水平高于无感染者。 OBJECTIVE To observe and compare the distribution of genotypes of E-selectin second and fourth exons and levels of serum inflammatory factors between the coronary heart disease patients and the coronary heart disease patients complicated with pulmonary infections.METHODS Totally 342 coronary heart disease patients complicated with pulmonary infections who were treated in hospitals from Jan 2015 to Mar 2017 were chosen as the study group,meanwhile,302 coronary heart disease patients who were not complicated with infection were set as the control group.The genetic polymorphisms of E-selectin second exon+G98 Tand fourth exon+A561 Cwere detected for the two groups of patients by using polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP),and the levels of serum inflammatory factors interleukin-6(IL-6),interleukin-8(IL-8),Creactive protein(CRP),procalcitonin(PCT)and E-selectin of the patients were detected by means of double antibody sandwich method.RESULTS There were significant differences in the genotypes of+G98 Tand+A561 Cbetween the two groups of patients(P〈0.05).The distribution of GG and GT,AA and AC genotypes was significantly higher in the study group than in the control group(P〈0.05).The levels of serum E-selectin,IL-6,IL-8,CRP and PCT were significantly higher in the study group than in the control group(P〈0.05).CONCLUSION There is significant difference in the distribution of genotypes of E-selectin second and fourth exons between the coronary heart disease patients complicated with pulmonary infections and the patients without infection.The serological levels of the coronary heart disease patients complicated with pulmonary infections are higher than those of the patients without infection.
