关节置换术后患者血清PCT与CRP水平变化趋势及其临床意义

Changes of levels of serum PCT and CRP after arthroplasty and their clinical value

目的分析关节置换术术后感染患者血清中降钙素原(PCT)和C-反应蛋白(CRP)水平的变化趋势,探讨其对术后感染诊断的临床价值。方法选择2014年2月-2017年2月在医院接受关节置换术治疗的81例患者作为研究对象,其中术后发生感染的26例患者作为感染组,55例未发生感染患者作为对照组,比较两组患者术前、术后1、4、7d血清CRP和PCT的水平。结果两组患者行关节置换术前PCT、CRP水平比较差异无统计学意义;两组患者术后1dPCT和CRP水平比较差异无统计学意义,感染组患者术后4、7dPCT和CRP水平均高于未感染组(P<0.05);不同类型的感染患者CRP水平比较差异无统计学意义,PCT水平比较差异有统计学意义(F=2.236,P=0.036)。结论联合检测关节置换术后患者血清PCT、CRP水平变化,对于临床诊断术后感染具有一定的价值。

OBJECTIVE To analyze the changes of serum procalcitonin（PCT）and C-reactive protein（CRP）of patients with infections after arthroplasty and explore their clinical value in diagnosis of postoperative infections.METHODS A total of 81 patients who received arthroplasty in the hospital from Feb 2014 to Feb 2017 were recruited as the study objects,26 patients who had postoperative infections were assigned as the infection group,55 patients who did not have infection were set as the control group.The levels of serum CRP and PCT were compared between the two groups of patients before the surgery and after the surgery for 1 day,4 days and 7 days.RESULTS There were no significant differences in the levels of PCT and CRP between the two groups of patients before the arthroplasty;there were no significant differences in the levels of PCT and CRP between the two groups of patients after the surgery for 1 day,the levels of PCT and CRP of the infection group were significantly higher than those of the non-infection group after the surgery for 4 and 7 days（P〈0.05）.There was no significant difference in the CRP level among the patients with different types of infections,and there was significant difference in the PCT level（F=2.236,P=0.036）.CONCLUSIONThe joint detection of the serum PCT and CRP has certain value in clinical diagnosis of postoperative infections in the patients after arthroplasty.
