益气消癥通络方治疗慢性阻塞性肺疾病稳定期疗效观察

Clinical observation on the treatment of chronic obstructive pulmonary disease stable phase with Yiqi Xiaozheng Tongluo Formula

目的:观察益气消癥通络方及益气化痰祛瘀方治疗慢性阻塞性肺疾病(COPD)稳定期患者的临床疗效。方法:将50例Ⅱ~Ⅲ级的COPD稳定期患者随机分层分为益气消癥通络组和对照组。益气消癥通络组在西医常规治疗基础上服用益气消癥通络方,对照组在西医常规治疗基础上服用益气化痰祛瘀方。疗程2个月,观察治疗前后肺功能、主要症状单项积分、中医证候疗效、CAT评分、m MRC评分等指标变化。结果:益气消癥通络组可改善COPD稳定期患者肺功能中FEV1%、MEF50%及MMEF75/25%(P<0.05),且FEV1%优于对照组(P<0.05);主要症状中喘憋积分益气消癥通络组改善优于对照组(P<0.05);益气消癥通络组在改善肺气虚证、血瘀证优于对照组(P<0.05),对照组改善脾气虚证优于益气消癥通络组(P<0.05)。结论:益气消癥通络方治疗COPD稳定期患者的临床疗效较益气化痰祛瘀方更优。

Objective： To observe the clinical efficacy of Yiqi Xiaozheng Tongluo Formula and Yiqi Huatan Quyu Formula in the treatment of chronic obstructive pulmonary disease（COPD） in stable phase. Methods： Fifty patients with stage Ⅱ~Ⅲ COPD were randomly divided into Yiqi Xiaozheng Tongluo group and control group. On the basis of routine treatment of western medicine, the Yiqi Xiaozheng Tongluo group was treated with Yiqi Xiaozheng Tongluo Formula, while the control group was treated with Yiqi Huatan Quyu Formula. During the course of 2 months, the changes of pulmonary function, cardinal symptom, TCM syndrome, CAT and m MRC were observed before and after treatment. Results： Yiqi Xiaozheng Tongluo group improved pulmonary function of COPD patients in FEV1%, MEF50%, MMEF75/25%（P〈0.05）, and FEV1% was better than the control group（P〈0.05）. Yiqi Xiaozheng Tongluo group was better than the control group in improving the cardinal symptom of dyspnea score（P〈0.05）. At the same time, Yiqi Xiaozheng Tongluo group was superior to the control group in improving syndrome of deficiency of lung qi and syndrome of blood stasis（P〈0.05）. The control group was better than Yiqi Xiaozheng Tongluo group in improving syndrome of deficiency of spleen qi（P〈0.05）. Conclusion： The clinical efficacy of Yiqi Xiaozheng Tongluo Formula in the treatment of patients with stable COPD is better than Yiqi Huatan Quyu Formula.
