摘要 目的探讨环氧化酶-2(cycloxygenase-2,COX-2)和基质金属蛋白酶-14(matrix metalloproteinase,MMP-14)在甲状腺癌中的表达及其临床意义。方法选取甲状腺外科患者85例,分为甲状腺癌组、甲状腺良性病变组和甲状腺体正常组。通过免疫组化切片染色法检验判定COX-2和MMP-14在甲状腺体中的表达情况。对甲状腺癌组肿瘤大小、临床分期和是否出现淋巴结转移的临床症状与COX-2和MMP-14表达情况进行相关性分析。结果 COX-2和MMP-14在甲状腺癌患者中阳性表达率最高,在甲状腺良性病变中阳性表达率次之,而在甲状腺体正常组织中不表达。COX-2和MMP-14水平与甲状腺癌的肿瘤直径、临床分期、有无淋巴结转移有关。结论 COX-2和MMP-14的表达与甲状腺癌的发生及其临床表现皆有明确的相关性,可作为临床上甲状腺癌的诊断诊治的有效指标。 Objective To investigate the expression of cyclooxygenase-2( COX-2) and matrix metalloproteinase-14( MMP-14) in thyroid carcinoma and its clinical significance. Methods A total of 85 patients in the department of thyroid surgery were divided into the thyroid carcinoma group,the benign thyroid lesion group and the normal group. The expression of COX-2 and MMP-14 in thyroid gland was determined by immunohistochemical staining. The correlation analysis was performed to the clinical symptoms of the tumor size,clinical stage and lymph node metastasis and the expression levels of COX-2 and MMP-14 in the thyroid carcinoma group. Results The positive expression rate of COX-2 and MMP-14 was the highest in thyroid carcinoma,followed by benign thyroid lesions while COX-2 and MMP-14 were not expressed in normal thyroid gland. The levels of COX-2 and MMP-14 were associated with tumor diameter,clinical stage,and lymph node metastasis in the thyroid carcinoma. Conclusion The expression of COX-2 and MMP-14 is closely related to the occurrence and clinical manifestations of thyroid carcinoma,which can be used as an effective index for the diagnosis and treatment of thyroid carcinoma.
