3DSTI、2DE和MRI检测扩张型心肌病患者左心室收缩功能结果的比较 被引量:4

Comparative study on the left ventricular systolic function by three dimensional speckle tracking imaging,two-dimensional echocardiography and magnatic resonance imaging in patients with dilated cardiomyopathy

摘要 目的:比较三维斑点追踪成像(3DSTI)、二维超声心动图(2DE)和磁共振成像(MRI)检测扩张型心肌病(DCM)患者左心室收缩功能的结果。方法:纳入22例DCM患者,在24h内分别运用MRI、3DSTI和2DE检测左室收缩末期容积(LVESV)、左室舒张末期容积(LVEDV)及左室射血分数(LVEF),分析比较各种检测方法所得结果之间的异同。结果:3种方法测得的LVESV、LVEDV和LVEF值组间比较均呈显著正相关。2DE组的LVEF值为(33.4±11.0)%,3DSTI组为(29.1±9.6)%,MRI组为(25.8±10.4)%,3组间差异有统计学意义(P<0.05);其中3DSTI组分别与2DE组和MRI组比较差异均无统计学意义(均P>0.05);MRI组较2DE组明显降低(P<0.05)。结论:在DCM患者中,与作为左室收缩功能检测金标准的MRI比较,3DSTI检测得到的LVEF值比2DE更为准确。 Objective:The three measurements for left ventricular systolic function by three dimensional speckle tracking imaging(3 DSTI),two-dimensional echocardiography(2 DE)and magnatic resonance imaging(MRI)were comparatively studied in patients with dilated cardiomyopathy(DCM).Method:End-diastolic volume(LVEDV),end-systolic volume(LVESV)and ejection fraction(LVEF)of left ventricle in 22 patients with DCM were measured using the three kinds of methods in 24 hours,and the results were compared and analyzed.Result:The LVEDV,LVESV and LVEF in the patients with DCM were obviously positive correlation among the three groups.The LVEF were(33.4±11.0)%,(29.1±9.6)% and(25.8±10.4)%in 2 DE,3 DSTI and MRI groups respectively(P〈0.05).The difference of LVEF between 3 DSTI and 2 DE groups was no statistical significance as well between 3 DSTI and MRI groups(P〉0.05),other than between 2 DE group and MRI group(P〈0.05).Conclusion:Compared with that of MRI,the golden standard of left ventricular systolic function detection,the result of LVEF detected using 3 DSTI is more accurate than that of 2 DE detection in patients of DMC.
