摘要
目的食管癌是一种常见的消化系统恶性肿瘤。食管癌包括食管鳞状细胞癌和食管腺癌,相关研究显示,食管癌在所有肿瘤发病率中排第5位,死亡率居第4位。本研究旨在通过Meta分析,探讨食管癌组织中HMGB1的表达与临床各病理参数之间的相关性。方法检索PubMed、ScienceDirect、OVID、Web of science、中国期刊全文数据库、万方数据库和维普数据库,2000-2016年发表的有关HMGB1与食管癌各病理参数之间相关性的文献,采用Revman 5.3对纳入的研究进行Meta分析。结果共检索到520篇相关文献,通过阅读文献摘要,然后阅读全文,按照纳入标准筛选出6篇文献,总计病例426例。Meta分析结果显示,HMGB1的表达水平与分化程度和肿瘤大小无相关性,P>0.05;而与TNM分期、淋巴结转移和病理类型为鳞癌相关,P<0.05。临床分期较晚(P=0.000 8)、发生淋巴结转移(P<0.000 1)、病理类型为鳞癌患者(P<0.000 1)的癌组织HMGB1的阳性表达率较高。结论食管癌患者癌组织HMGB1的表达情况与患者临床分期、病理类型及是否有淋巴结转移密切相关,有望成为判定食管癌侵袭、转移的重要标志物。
OBJECTIVE Cancer of the esophagus is a common malignant tumor of the digestive system.Esophageal cancer includes squamous cell carcinoma of the esophagus and adenocarcinoma of the esophagus.The related articles show that the incidence of esophageal cancer is fifth in all the incidence of cancer,and the mortality rate is fourth.Many studies have shown that HMGB1 plays a key role in the development of esophageal cancer.This study was intended to analyze the correlation between the expression of HMGB1 and the clinical pathology parameters in esophageal cancer tissues through meta-analysis.METHODS The databases of PubMed,ScienceDirect,OVID,Web of science,CNKI,Wanfang and VIP were retrieved for collecting the articles about the relationship between the expression of HMGB1 and clinical pathological factors of esophageal cancer tissues from 2000 to 2016.The extracted data was analyzed by Revman 5.3 software.RESULTS The orginal study yielded 520 relevant articles.Six studies were finally included,involving 426 esophageal cancer patients.The results showed that the expression level of HMGB1 was not correlated with the degree of differentiation and tumor size(P〈0.05).It was associated with TNM staging,lymph node metastasis and squamous carcinoma(P〈0.05).Tumor tissues with advanced clinical stage(P=0.000 8),lymph node metastases(P0.000 1)and squamous carcinoma(P0.000 1)is of higher HMGB1 positive expression.CONCLUSIONS The positive expression of HMGB1 is associated with clinical stage and pathological type,lymph node metastasis.HMGB1 is promising to serve as an indicator to evaluate the progression and metastasis.
