足三里穴电针恢复脑卒中患者胃肠功能的临床观察

Zusanli Electroacupuncture on Clinical Effect of Promoting the Recovery of Gastrointestinal Function in Stroke Patients

[目的]观察电针足三里治疗脑卒中继发胃肠功能紊乱患者的疗效。[方法]选取2015年5月至2016年6月期间在菏泽市中医医院脑病科收治的80例符合诊断标准的脑卒中继发胃肠功能紊乱患者,根据患者的病历号顺序随机分为观察组(n=40)和对照组(n=40)。对照组服用马来酸曲美布汀片促进胃肠功能恢复,观察组在对照组的基础上给予电针双侧足三里穴。观察患者胃肠功能症状的改变情况,测定其肠鸣音、排便排气恢复至正常状态的时间,检测血清胃泌素(Gastrin,GAS)、胃动素(Motilin,MOT)含量。[结果]经2个完整连续疗程的治疗后,观察组有效率为90.0%,对照组为70.0%,两组间差异有统计学意义(P<0.05)。与对照组相比,观察组患者的肠鸣音恢复正常所需时间,排便、排气恢复正常状态所需时间均明显缩短(P<0.05),并且观察组患者GAS、MOT含量显著升高,两组间差异有统计学意义(P<0.05)。[结论]电针足三里穴可有效改善脑卒中继发胃肠功能紊乱的临床疗效,通过提升胃肠运转的动力,增强胃肠激素GAS、MOT的分泌,从而促进胃肠功能的恢复。

[Objective] To evaluate the therapeutic effect of electroacupuneture at ＂Zusanli＂ in the treatment of patients with secondary gastrointestinal dysfunction after stroke.[Methods] From May 2015 to June 2016, 80 patients with gastrointestinal dysfunction who met the diagnostic criteria were enrolled in the Department of Encephalopathy, Heze Hospital of Traditional Chinese Medicine. They were randomly divided into observation group（n=40） and control group（n=40） according to their medical records.Control patients taking trimebutine maleate tablets to promote gastrointestinal function recovery, and the observation group on the basis of the control group were given electroacupuncture Zusanli points.The changes of gastrointestinal function were observed, the bowel sounds and defecation exhaust time were measured, and the levels of serum gastrin（GAS） and motilin（MOT） were measured. [Results] After two complete and consecutive courses of treatment, the effective rate of the observation group was 90.0% and that of the control group was 70.0%. The difference between the two groups was statistically significant（P〈0.05）. Compared with the control group, the bowel sounds time and defecation exhausting time in the observation group were significantly shorter（P〈0.05）, and the levels of GAS and MOT in the observation group were significantly increased（P〈0.05）. [Conclusion] Zusanli electroacupuncture can effectively improve the clinical efficacy of gastrointestinal dysfunction after stroke, and improve gastrointestinal function by enhancing the power of gastrointestinal movement and enhancing secretion of GAS and MOT of gastrointestinal hormones.
