促性腺激素释放激素激动剂对腹腔镜卵巢囊肿剔除术患者血清激素水平及基质血流的影响

Effects of gonadotropin-releasing hormone agonist on serum hormone levels and stromal blood flow in patients underwent laparoscopic ovarian cystectomy

目的探讨促性腺激素释放激素激动剂(GnRH-a)对腹腔镜卵巢囊肿剔除术患者血清激素水平及基质血流的影响。方法卵巢囊肿患者116例随机均分为观察组和对照组。两组均实施腹腔镜下卵巢囊肿剔除术。观察组术后在月经来潮后第1天给予戈舍瑞林3.6mg皮下注射,每4周1次,共注射4次;对照组术后不用GnRH-a。随访6个月,比较两组血清激素水平和基质血流情况。结果随访6个月,观察组血清卵泡刺激素、促黄体生成素、雌二醇含量均低于对照组(P<0.05或P<0.01);舒张末期最低血流速度、收缩期峰速高于对照组(P<0.01);月经正常(84.48%vs.65.52%)、恢复排卵比例(81.03%vs.58.62%)高于对照组,而复发率低于对照组(5.17%vs.17.24%)(P<0.05)。结论 GnRH-a有助于调节腹腔镜卵巢囊肿剔除术患者血清激素水平,改善基质血流,提高治疗效果。

Objective To investigate the effects of gonadotropin-releasing hormone agonist（GnRH-a）on serum hormone level and stromal blood flow in the patients underwent laparoscopic ovarian cystectomy.Methods One hundred and sixty-one patients with ovarian cysts were equally randomized into two groups.The patients in group A received laparoscopic ovarian cystectomy and given subcutaneous injection of goserelin 3.6 mg after first postoperative menses,which was repeated every other 4 weeks for 3 times.The patients in group C received laparoscopic ovarian cystectomy without injection of goserelin.The patients were followed-up for 6 months and serum hormone levels and stromal blood flow were compared between two groups.Results Serum levels of follicle stimulating hormone,luteinizing hormone and estradiol were lower in group A than those in group C（P〈0.05 or P〈0.01）.The end-diastolic minimum velocity and systolic peak velocity were higher in group A than those in group C（P〈0.01）.The percentages of the patients with the recovery of normal menstruation and ovulation were higher in group A than those in group C（84.48% vs.65.52% and81.03% vs.58.62%）（P〈0.05）.The recurrence rate was lower in group A than that in group C（5.17% vs.17.24%）（P〈0.05）.Conclusion GnRH-a is helpful in regulating serum hormone levels,improving the stromal blood flow and enhancing the therapeutic effect in the patients underwent laparoscopic ovarian cystectomy.
