摘要
目的运用血栓弹力图仪(TEG)评估定期血小板单采对献血者凝血和血小板功能的影响。方法选取2016年8月—2017年7月本中心定期单采血小板捐献者105例作为单采组(既往1年内献血次数≥6次的献血者),并根据单采频次分为3个亚组:6—11次组,12—17次组,18—24次组。另随机选取初次血小板捐献者32例作为对照组。血液标本在血小板单采前采集,凝血功能和血小板功能分析采用Haemonetics公司血栓弹力图仪,血小板常规采用SYSMEX公司XS-800i血细胞分析仪。结果单采组R时间、K时间、α角、MA值、Plt、MPV和PDW均在正常范围内,与对照组比较,差异均无统计学意义(P〉0.05);不同单采频次亚组间TEG参数(R时间、K时间、α角、MA)和血小板常规检测指标(Plt、MPV和PDW)比较,差异也均无统计学意义(P〉0.05);单采血小板频次与R、K、α、MA、PLT、MPV、PDW均无显著相关性(P〉0.05)。结论血栓弹力图仪可以系统、准确、全面地评估单采血小板献血者的凝血功能和血小板功能,定期血小板单采对献血者凝血和血小板功能没有显著影响。
Objective The aim of this study is to systematically evaluate the effect of regular plateletpheresis on the coagulation and platelet function of donors by thrombelastography( TEG).Methods We selected 105 cases of regular plateletpheresis donors in Zhejiang blood center dating from August 2016 to July 2017 as the regular donation group,which was further divided into 3 subgroups based on donation counts: 6—11,12—17 and 18—24 donations annually. We also selected 32 cases of first-time plateletpheresis donors as the control group. The blood samples were collected before donation. Coagulation and platelet functions were tested using TEG( Haemonetics company,Braintree,MA,USA). Platelet counts were tested by a SYSMEX XS-800 i blood cell analyzer.Results R time,K time,α-Angle,MA value、PLT、MPV and PDW of the regular donation group were all within the normal ranges. Compared with the control group,All indexes had no statistical significance( P〈0. 05). All parameters were not statistically significant among the three subgroups( P〈0. 05). The correlation between plateletpheresis frequency and R、K、α-Angle、MA、PLT、MPV、PDW values was not statistically significant.Conclusion The thrombelastography can assess the coagulation and platelet function of regular plateletpheresis donors systematically,effectively and accurately. Regular plateletpheresis had no significant impact on the coagulation and platelet function of donors.
