临床药师参与258例会诊病例分析及典型案例介绍

Clinical pharmacists' participation in case consultation:a retrospective study of 258 cases and typical case reviews

目的了解临床药师参与会诊的落实情况和效果,为更好地开展药学服务提供依据。方法对临床药师参与258例患者会诊情况,包括申请会诊的科室、病例基本情况、申请会诊目的、会诊形式、会诊建议是否采纳、治疗转归、检出的菌群等进行统计分析,并介绍典型案例。结果申请会诊科室中以神经外科病区最多(15.9%,41/258),其次为儿科普通病区(7.0%,18/258);申请会诊目的以调整抗感染治疗方案为主(94.6%,244/258);会诊意见全部采纳199例(77.1%),部分采纳35例(13.6%),未采纳24例(9.3%)。意见采纳后经治疗,好转197例(84.2%),无效37例(15.8%)。结论临床药师通过会诊融入到医疗团队,对合理用药起到了推动和促进作用。

Objective To summarize clinical pharmacists＇ participation in the consultation and to provide reference for the rational use of drugs and further development of clinical pharmacists. Methods The pratice of clinical pharmacists participating in 258 cases of clinical consultation was analyzed following aspects： departments of consultation, patient information, purpose of consultation, consultation mode, acceptance of consultation, treatment outcome, pathogen distribution and typical cases reviews. Results Most consultations occurred in the Neurosurgical Ward （15.9%, 41/258）, followed by general pediatrics （7%, 18/258）. The main purpose was to adjust or guide the use of antibiotics （94.6%, 244/258）. Totally 199 consultation comments （77.1%） were taken completely, 35 （13.6%） were partly adopted, while 24 （9.30%） were refused. After the consultation, 197 patients were improved （84.2%）, while 37 remained unchanged （15.8%）. Conclusion The participation of clinical pharmacists in the consultation has became an important part of medical treatment, and plays positive role in the rational use ofdrues.
