自拟清肺汤联合茶碱治疗支气管哮喘对肺功能保护及IgE、NO、ET水平影响 被引量:5

Effects of Self-made Qingfei Decoction Combined with Theophylline on the Lung Function and the Levels of IgE,NO and ET in Patients with Bronchial Asthma

摘要 目的:观察研究自拟清肺汤联合茶碱治疗支气管哮喘对肺功能保护及Ig E、NO、ET水平影响。方法:选取2014年6月1日—2015年12月1日接收治疗的支气管哮喘患者150例,按照随机数表法分为观察组和对照组两组,观察组采用自拟清肺汤联合茶碱治疗,对照组仅采用茶碱治疗,观察比较两组患者临床疗效,肺功能1秒率(FEV1/FVC)、1秒用力呼气容积与预计值百分比(FEV1)、呼气峰值流速(PEF),哮喘控制测试量表(ACT)评分以及血清Ig E、内皮素(ET)、一氧化氮(NO)的变化情况。结果:观察组患者总有效率(92.00%)显著高于对照组(80.00%),比较有统计学意义(P〈0.05),治疗前,两组患者肺功能情况比较差异较小,无统计学意义(P〉0.05),治疗后,两组患者肺功能FEV1、FEV1/FVC、PEF均有所上升,但是观察组FEV1(77.24±5.04)%显著高于对照组(66.73±4.34)%,观察组FEV1/FVC(70.63±7.34)%显著高于对照组(63.37±6.46)%,观察组PEF(72.12±4.43)mL/s显著高于对照组(67.23±3.98)mL/s,两组比较有统计学意义(P〈0.05),治疗前,两组患者ACT评分无较大差异(P〉0.05),治疗后,两组患者评分均有所上升,但观察组(25.12±1.57)分显著高于对照组(19.47±1.78)分,两组比较有统计学意义(P〈0.05),治疗前,两组患者的NO、ET水平差异较小(P〉0.05),治疗后,两组患者NO、ET水平均有降低,但观察组NO(69.36±5.79)μmol/L显著低于对照组(84.27±8.38)μmol/L,观察组ET(25.39±7.38)ng/L显著低于对照组(35.87±8.28)ng/L,两组比较有统计学意义(P〈0.05),治疗前,两组患者血清Ig E水平比较差异较小,无统计学意义(P〉0.05),治疗后,两组患者血清Ig E水平均有下降,但观察组Ig E(57.38±8.04)U/mL显著低于对照组(78.32±10.78)U/mL。结论:自拟清肺汤联合茶碱治疗支气管哮喘能够有效提高患者的临床疗效,显著提高患者ACT评分,降低患者Ig E、NO、ET水平,对肺功能保护有积极的作用。 Objective :To observe the effect of Self-made Qingfei Decoction combined with theophylline on the lung function and the levels of Ig E,NO and ET in the treatment of bronchial asthma. Methods :From June 1,2014—2015 year in December 1 st in our hospital received 150 patients with bronchial asthma treatment were randomly divided into observation group and control group two groups,the observation group was treated with Qingfei Decoction combined with theophylline,control group was only with theophylline,to observe the clinical curative effect of two groups,pulmonary function second rate(FEV1/FVC)and forced expiratory volume and percentage of predicted value(FEV1),peak expiratory flow rate(PEF),asthma control test scale(ACT)score and serum Ig E,endothelin(ET)and nitric oxide(NO)changes. Results :The total efficiency of the observation group(92%)was significantly higher than the control group(80%),there was statistically significant difference(P〈0.05),before treatment,two groups of patients with pulmonary function had little difference,no statistical significance(P〈0.05),after treatment,two groups of pulmonary function in patients with FEV1,FEV1/FVC and PEF were increased. But the observation group FEV1(77.24%±5.04%)was significantly higher than the control group(66.73%±4.34%),the observation group FEV1/FVC(70.63%±7.34%) was significantly higher than the control group(63.37%±6.46%),the observation group PEF(72.12±4.43)mL/s was significantly higher than the control group(67.23±3.98)mL/s,the comparison between the two groups had statistical significance(P〈0.05),before treatment,no significant difference between the two groups of patients with ACT score(P〈0.05),after treatment,scores of patients in two groups were increased,but the observation group(25.12±1.57)points was significantly higher than that of the control group(19.47±1.78)points,the two groups was statistically significant(P〈0.05),before treatment,the difference of NO,ET level of two groups of patients with small(P〈0.05),after treatment,two patients in group NO and ET were decreased,but the observation group NO(69.36±5.79)μmol/L was significantly lower than the control group(84.27±8.38)μmol/L the observation group,ET(25.39±7.38)ng/L was significantly lower than the control group(35.87±8.28)ng/L,the two groups was statistically significant(P〈0.05),before treatment,the serum Ig E level in patients with small difference between the two groups,no statistical significance(P〈0.05). After treatment,the average decline of two groups of patients with serum Ig E water. But the observation group Ig E(57.38±8.04)U/mL was significantly lower than the control group(78.32±10.78)U/mL. Conclusion :Self-made Qingfei Decoction combined with theophylline in treating bronchial asthma can effectively improve the clinical curative effect of the patients,improve the patients ACT score,Ig E,NO,ET in patients with lower level,have a positive effect on lung function protection.
