逐瘀化痰法治疗下肢动脉硬化闭塞症临床研究

Clinical Study of Zhuyu Huatan Method in Treatment of Lower Extremity Arteriosclerosis Obliterans

目的：观察逐瘀化痰法治疗下肢动脉硬化闭塞症临床疗效及其对血脂水平和动脉硬化指标的影响。方法：将80例下肢动脉硬化闭塞症患者随机分为观察组（40例）、对照组（40例）,对照组予常规降糖、降压、降脂、抗血小板、血管扩张治疗,观察组在对照组治疗基础上以逐瘀化痰方口服,两组治疗前后下肢彩超检测踝-肱指数（ABI）、趾-肱指数（TBI）及足背动脉血流量,检测血脂中总胆固醇（TC）、甘油三酯（TG）、高密度脂蛋白（HDL-C）、低密度脂蛋白（LDL-C）、血清同型半胱氨酸（Hcy）含量并评价临床疗效。结果：观察组疗效优于对照组（P〈0.05）;治疗后,观察组与对照组TG、TC、LDL-C及Hcy水平均降低（P〈0.05）,观察组低于对照组（P〈0.05）;HDL-C水平升高（P〈0.05）,观察组高于对照组（P〈0.05）;治疗后,观察组ABI、TBI指数及足背动脉血流量均高于治疗前（P〈0.05）,观察组高于对照组（P〈0.05）。结论：逐瘀化痰法治疗下肢动脉硬化闭塞症可有效改善血脂水平及动脉硬化,值得临床推广使用。

Objective ：To observe the clinical curative effect of Zhuyu Huatan method on lower extremity arteriosclerosis obliterans and its influence on blood lipid level and arteriosclerosis index. Methods ：80 cases of lower extremity arterial occlusive disease were randomly divided into observation group（40 cases）and control group（40 cases）,the control group was received routine hypoglycemic,lipid-lowering,antihypertensive,antiplatelet,vasodilator therapy. Observation group in the control group was treated with Zhuyu Huatan Decoction,to detect the two groups before and after treatment of lower extremity ultrasound detection ankle brachial index（ABI）and toe brachial index（TBI）and dorsal artery blood flow,the clinical curative effect of serum lipids,serum homocysteine content and evaluation. Results ：The curative effect of the observation group was better than that of the control group（P〈0.05）. After treatment,the observation group and the control group TG,TC,LDL-C and Hcy levels decreased（P〈0.05）,the observation group was lower than that of the control group（P〈0.05）;HDL-C levels were elevated（P〈0.05）,the observation group was higher than that of the control group（P〈0.05）;After treatment,the ABI,TBI index and the blood flow volume of the dorsalis pedis artery in the observation group were higher than those before treatment（P〈0.05）,the observation group was higher than that of the control group（P〈0.05）. Conclusion ：Zhuyu Huatan method in the treatment of lower extremity arteriosclerosis obliterans can effectively improve blood lipid level and arteriosclerosis,and is worthy of clinical use.
