ICU护士创伤后应激障碍与心理一致感及反刍性沉思的相关性

Correlation between post traumatic stress disorder and psychological coherence,ruminative meditation in ICU nurses

目的探讨重症监护病房(ICU)护士创伤后应激障碍(PTSD)与心理一致感及反刍性沉思的相关性。方法采用方便抽样法,选取长沙市8所三级甲等综合医院和海南省1所二级甲等综合医院共219名ICU护士作为调查对象,应用一般资料问卷、PTSD症状清单平民版(PCL-C)、心理一致感表(SOC)及简体中文版事件相关反刍性沉思问卷(C-ERRI)对其进行横断面调查。结果 ICU护士PTSD总分为28.98±9.21,阳性率为18.7%,且在不同文化程度、职称和工作医院的分组得分差异有统计学意义(P<0.05)。经Pearson相关性分析,护士PTSD得分与心理一致感得分呈负相关(P<0.01),与侵入性反刍性沉思和目的性反刍性沉思得分均呈正相关(P<0.01)。结论 ICU护士PTSD的发生率处于较高水平,需关注ICU护士PTSD,引导其进行积极的的反刍性沉思,提高心理一致感水平,采取正确的应对措施,以降低护士的PTSD水平。

[Objective]To explore the correlation between post traumatic stress disorder（PTSD） and psychological coherence,ruminative meditation in ICU nurses.[Methods]By using the random sampling method,219 ICU nurses were selected from eight grade-A tertiary general hospitals in Changsha City and one grade-A secondary general hospital in Hainan Province as the research object. The general information questionnaire,PTSD checklist civilian Version（PCL-C）,sense of coherence scale（SOC）and simplified version of Chinese event-related rumination rumination questionnaire（C-ERRI） were employed in the crosssectional survey.[Results]The total score of PTSD in ICU nurses was 28.98 ±9.21 with the positive rate of 18.7%,there were statistically significant differences in the scores among different groups in educational level,professional title and working hospital（P〈 0.05）. Pearson correlation analysis showed that there was a negative correlation between score of PTSD and score of psychological coherence（P 0.01）,while the score of PTSD was positively correlated with the scores of invasive ruminative meditation and purposeful ruminative meditation（P〈0.01）.[Conclusion]The incidence of PTSD in ICU nurses is at a high level.It is necessary to pay attention to PTSD in ICU nurses,guide ICU nurses to carry out positive ruminant meditation,improve the level of psychological consistency,and take correct measures to reduce the PTSD level in nurses.
