摘要
目的探讨纤维蛋白原(FIB)、D-二聚体(D-D)和尿B2微球蛋白(B2-MG)与过敏性紫癜(HSP)及过敏性紫癜性肾炎(HSPN)发病的关系与相关性,评价其在儿童HSP早期肾损害的临床价值。方法选择HSP患儿50例,HSPN患儿38例,健康对照组40例,分别测定血FIB、D-D和β2-MG水平。结果HSPN组血FIB、D-D和β2-MG水平均高于HSP组和健康对照组,差异有统计学意义(P〈0.05)。HSP组血FIB、D-D和β2-MG水平均高于健康对照组,差异有统计学意义(P〈0.05)。血FIB、D-D与β2-MG水平均呈正相关(P〈0.05)。结论HSP患儿存在凝血、纤溶系统异常,可能参与本病的肾损害。血FIB、D-D和β2-MG可以作为早期监测儿童HSP肾损害的检测指标。
Objective To explore the relationship and correlation between fibrinogen、 D- dimer, β2-microglobulin and allergic purpura, Henoch Schonlein purpura nephritis. Evaluate the clinical value of early renal damage in children with Henoch Schonlein purpura. Methods 50 children with Henoch Schonlein purpura, Henoch Schonlein purpura nephritis ( n=38 ) and healthy control group ( n=40 ) were enrolled in this study. The levels of fibrinogen, D-timer, and Urine β2-Microglobulin were detected. Results The levels of fibrinogen, D-direct and Urine β2-Microglobulin in patients with Henoch Sehonlein purpura nephritis group were higher than those in allergic purpura group and healthy control group, the difference was statistically significant ( P〈0.05 ) . The levels of fibrinogen, D-dimer and Urine β2-Microglobulin were significantly higher in patients with Henoch Schonlein purpura than those in healthy controls ( P〈0.05 ) . The levels of fibrinogen and D-dimer were positively correlated with the level of Urine β2-Microglobulin ( P〈0.05 ) . Conclusion Children with Henoch Schonlein purpura have abnormal coagulation and fibrinolysis system, which may be involved in renal damage. Fibrinogen, D-dimer and Urine β2-Microglobulin can be used as early detection of renal damage in children with Henoch Schonlein purpura.
