微型腹腔镜与传统开放性手术治疗小儿鞘膜积液的临床疗效对比

Comparison of Clinical Efficacy of Microlaparoscopy and Traditional Open Surgery in the Treatment of Infantile Hydrocele

目的探究微型腹腔镜与传统开放性手术治疗小儿鞘膜积液的临床疗效。方法选取2016年4月-2017年3月收治的68例小儿鞘膜积液,随机分为观察组和对照组各34例,其中对照组予以传统开放性手术治疗,观察组予以微型腹腔镜,观察对比两组围术期观察指标、并发症及复发率。结果两组近期手术治愈率均为100%。观察组的手术时间、术中出血量、术后住院时间均少于对照组(P<0.05)。观察组术后并发症率为2.9%,低于对照组的20.6%(P<0.05)。术后随访6个月,观察组无复发,对照组复发率为17.6%(P<0.05)。结论微型腹腔镜与传统开放性手术治疗小儿鞘膜积液的疗效相似,但微型腹腔镜手术具有创伤小、术后并发症少、复发率低等优势,更利于在小儿外科中推广应用。

Objective To explore the clinical efficacy of microlaparoscopy and traditional open surgery in the treatment of children＇s hydrocele. Methods 68 cases of children with hydrocele from April 2016-2017 year in March, were randomly divided into observation group and control group with 34 cases in each group,the control group was treated with traditional open surgery,observation group received laparoscope,comparison of two groups of perioperative observation indexes, complications and recurrence rate. Results The cure rate of the two groups was 100%.The operation time of the observation group, the amount of bleeding in the operation,and the time of postoperative hospitalization were less than those of the control group（P〈0.05）.The postoperative complication rate in the observation group was 2.9%, which was lower than that of the control group 20.6%（P〈0.05）.After 6 months of follow-up, no recurrence was found in the observation group, and the recurrence rate of the control group was 17.6%（P〈0.05）. Conclusion Mini laparoscopic surgery is similar to traditional open surgery in the treatment of pediatric hydrocele.However,mini laparoscopic surgery has advantages of less trauma,fewer postoperative complications and lower recurrence rate,which is more conducive to the popularization and application in pediatric surgery.
