脑型肝豆状核变性合并椎动脉型颈椎病患者脑彩超监测情况研究 被引量:3

摘要 目的:分析脑型肝豆状核变性(WD)合并椎动脉型颈椎病患者颈部及颅内血管彩色多普勒超声检测结果,明确患者颈部及颅内中小动脉的血流动力学状态。方法:对23例正常对照组和30例住院的脑型肝豆状核变性合并椎动脉型颈椎病患者采用彩色多普勒超声检测其颈部及颅内中小动脉的内径、管壁厚度、血流色彩、血流频谱及速度、阻力指数等参数,通过对比分析,明确脑型肝豆状核变性合并椎动脉型颈椎病患者头颈部中小动脉的血流动力学状态。结果:脑型WD合并椎动脉型颈椎病患者右侧颈总动脉平均血流速度低于正常对照组,双侧大脑前、中、后动脉平均血流速度均低于正常对照组,左椎动脉颅内段平均血流速度低于正常对照组,差异均有统计学意义;脑型WD合并椎动脉型颈椎病患者左侧椎动脉颅内段平均血流速度低于颈段(P<0.05),而正常对照组无显著差异。结论:脑型WD合并椎动脉型颈椎病患者头颈部中小动脉的血流动力学状态是异常的,其颅内及颈部部分中小动脉平均血流速度低于正常对照组。 Objective: The results of color Doppler ultrasonography of cervical and intracranial vessels in patients with cerebral hepatolentieular degeneration (WD) combined with vertebral artery type cervical spondylosis were ana- lyzed, and the hemodynamic state of the patient' s neck and intraeranial arterioles was clearly identified. Methods: The diameter, wall thickness, and blood volume of the cervical and intracranial arterioles were measured in 23 normal con- trols and 30 hospitalized patients with cerebral hepatolenticular degeneration and vertebral artery type cervical spondy- losis using color Doppler ultrasound. Flow color, blood flow spectrum and speed, resistance index and other parameters. Through comparative analysis, the hemodynamie state of the head and neck mid-small arteries in patients with cerebral hepatolentieular degeneration combined with vertebral artery type cervical spondylosis was identified. Results: The mean blood flow velocity of the fight common carotid artery in cerebral WD patients with vertebral artery type cervical spondylosis was lower than that in the normal control group. The mean blood flow velocity of the anterior, middle and posterior cerebral arteries in the bilateral WD was lower than that in the normal control group. The left vertebral artery The average blood flow velocity in the intraeranial segment was lower than that in the normal control group, and the dif-ferences were statistically significant. The average blood velocity of intracranial segments of the left vertebral artery in patients with cerebral WD combined with vertebral artery type cervical spondylosis was lower than that in the cervical segment (P〈0.05), but there was no significant difference in the normal control group. Conclusion: The hemodynamic status of the head and neck mid-small arteries in patients with cerebral WD combined with vertebral artery type cervi- cal spondylosis is abnormal, and the average blood flow velocity in the intracranial and cervical middle and small arter- ies is lower than that in the normal control group.
