2014年-2016年腹泻病患者沙门菌分离株耐药性分析

Drug resistance analysis of Salmonella in diarrhea patients during 2014-2016

目的分析腹泻患者中沙门菌分离株的耐药特征。方法收集2014年-2016年闵行区腹泻病监测点腹泻患者沙门菌分离株,用纸片法测定沙门菌对环丙沙星、头孢噻肟、头孢呋辛、头孢西丁、左氧氟沙星、诺氟沙星、庆大霉素、氯霉素、复方新诺明、四环素、萘啶酸的耐药性。结果 489株沙门菌分布于32种血清型,常见血清型为肠炎沙门菌(31.1%)、鼠伤寒沙门菌(25.2%)和吉伟沙门菌(8.6%)。沙门菌对头孢噻肟、头孢呋辛、氯霉素、四环素、复方新诺明、庆大霉素和萘啶酸共7种抗生素的耐药率呈逐年增高的趋势。耐药血清型主要为鼠伤寒沙门菌、吉伟沙门菌和肠炎沙门菌。男性腹泻患者中分离的沙门菌对萘啶酸耐药率高于女性。结论加强沙门菌对抗生素的耐药性监测,随时更新抗生素耐药信息,对腹泻病防控以及临床合理选用抗生素具有重要参考意义。

Objective To analyze the drug resistance of Salmonella isolates in diarrhea patients. Methods The Salmonella isolates were collected from diarrhea patients in surveillance sites of Minhang District during 2014-2016,so as to detect the drug resistance of Salmonella to antibiotics including ciprofloxacin,cefotaxime,cefoxitin,cefuroxime,levofloxacin,norfloxacin,gentamicin,chloramphenicol,cotrimoxazole,tetracycline,nalidixic acid by using KB method. Results The 489 strains of Salmonella covered 32 different serotypes,with Salmonella enteritidis（ 31. 1%）,Salmonella typhimurium（ 25. 2%） and Salmonella give（ 8. 6%） the dominant. The Salmonella strains showed the increasing resistance to seven antibiotics including cefotaxime,cefoxitin,chloramphenicol,tetracycline,cotrimoxazole,gentamicin and nalidixic acid year by year. The predominant serotypes of Salmonella resistant to antibiotics were Salmonella typhimurium,Salmonella enteritidis,Salmonella give. The resistant rate of Salmonella isolated from male patients was significantly higher that from female patients. Conclusion It is important to strengthen the surveillance of antimicrobial resistance of Salmonella,to update the profile of resistance for diarrhea prevention and to rationally use antibiotics.
