摘要
目的:观察针刺七星穴结合透穴治疗中风后肩关节半脱位的临床疗效。方法:将52例中风后肩关节半脱位患者随机分配为2组各26例。2组均采用中风常规针刺治疗方案,在此基础上,对照组给予常规针刺治疗,以及运动疗法、作业治疗等。治疗组在对照组治疗方案的基础上加用针刺七星穴结合透穴法治疗,治疗4周后,评定2组患者肩痛改善程度,肩关节主动前屈、主动外展活动度。结果:2组肩痛临床疗效比较,总有效率治疗组92.3%,对照组65.4%,2组间比较,差异有统计学意义(P<0.05)。2组FMA评分主动前屈、主动外展评分分别与治疗前比较,差异均有统计学意义(P<0.05)。治疗后,2组主动前屈、主动外展评分组间分别比较,差异均有统计学意义(P<0.05)。结论:加用针刺七星穴结合透穴法治疗中风后肩关节半脱位较只用常规疗法能有效改善肩痛、肩关节前屈、外展活动度,其临床疗效更明显。
Objective:To observe the clinical effect of acupuncture at Qixing point combined with through point for shoulder subluxation after stroke.Methods:Divided 52 cases of patients with shoulder subluxation after stroke into two groups randomly,26 cases in each group.Both groups received the routine treatment of acupuncture after stroke,while the control group was additionally given routine acupuncture,exercise therapy and occupational therapy for treatment.The treatment group additionally received modified acupuncture at Qixing point combined with through point for treatment.After four weeks of treatment,evaluated the improvement degree of shoulder pain and activity degrees of shoulder joint active flexion and active abduction.Results:Compared the clinical effect of shoulder pain in the two groups,the total effective rate was 92.3% in the treatment group and was 65.4% in the control group,difference being significant(P〈0.05).Compared the scores of active abduction and active flexion of Fugl-Meyer Assessment(FMA)in the two groups with those before treatment,differences being significant(P〈0.05).After treatment,there were significant differences being found in the intergroup comparisons of scores of active abduction and active flexion in the two groups(P〈0.05).Conclusion:The therapy of modified acupuncture at Qixing point combined with through point method for shoulder after stroke can improve shoulder pain,shoulder flexion,and abduction activity degree more effectively and its clinical effect is more obvious when compared with the simple routine therapy.
