补肾坚骨汤对老年人牙周炎疗效的临床研究

The clinical effect of“BU SHEN JIAN GU TANG” on periodontitis in the elderly

目的:评价补肾坚骨汤联合牙周基础治疗对老年人中重度牙周炎的临床治疗效果。方法:40例患有中重度慢性牙周炎的老年患者随机分为2组,中药治疗组(用药组)行牙周基础治疗同时口服补肾坚骨汤4周,对照组仅行牙周基础治疗,于基线、治疗4周后即刻抽血检查血常规、肝肾功能,记录不良反应发生情况;于基线及治疗8周后采集各组牙周指标包括观察牙的探诊深度(probing depth/PD)、附着丧失(attachment loss/AL)、菌斑指数(plaque index/PLI)、出血指数(bleeding index/BI)及大于等于5mm深牙周袋位点的探诊出血(bleeding on pr obing/BOP)百分比,将两组治疗前后各项指标结果进行比较分析,并对比两组不良反应发生情况及药物对肝肾的毒性反应。结果:治疗前两组牙周临床各指标数值经统计学检验无明显差异;两组在治疗后8周时的牙周各指标值与基线值比较均显著下降(P<0.01);用药组治疗后PD、BI及≥5mm深牙周袋位点的BOP%明显低于对照组(P<0.05,P<0.01),治疗后两组PLI及AL无显著差异;服药组患者未出现肝肾异常及不良反应。结论:补肾坚骨汤能减轻老年人牙周炎患者牙周组织炎症和出血,且无明显的毒副作用,对老年人牙周炎有积极的辅助治疗作用。

Objective：To evaluate the clinical effect of＂BU SHEN JIAN GU TANG＂combined with periodontal basic therapy on the elderly patients with moderate and severe periodontitis.Methods：Forty elderly patients with moderate and severe chronic periodontitis were randomly divided into two groups.The traditional Chinese medicine treatment group（medication group）received periodontal basic treatment and BU SHEN JIAN GU TANG for 4 weeks,while the control group only received periodontal basic treatment.Blood routine and Liver and kidney function parameters were detected and analyzed in the venous blood of all patients at baseline and four weeks after treatment.Adverse reactions were recorded.Clinical evaluation of periodontal parameters including probing depth（PD）,attachment loss（AL）,plaque index（PLI）,bleeding index（BI）and bleeding on probing（BOP）was conducted at baseline and at 8 weeks after treatment.Results：There were no statistical significances in all periodontal parameters between two groups at baseline.Obvious clinical improvement was exhibited in all patients after treatment（P〈0.01）.There were significant reductions in PD and BI in all sites and in PD and BOP%in deep sites greater than or equal to 5mm.No kidney or liver dysfunctions and adverse reactions were revealed in herbal treatment group.Conclusions：＂BU SHEN JIAN GU TANG＂can alleviate periodontal tissue inflammation and bleeding and have no adverse effects in periodontitis in the elderly,so that it may have an active therapy for periodontal disease.
