基于胆囊切除术病例分析我国住院费用结构变化

Analysis on the Changes of Hospitalization Expenses Structure Based on the Cases of Cholecystectomy in China in Recent Years

目的:通过胆囊切除术病例,了解2013─2016年我国住院费用结构的发展变化,为我国进一步推进医疗服务价格和支付制度改革提供参考。方法:采用环比、定基比描述胆囊切除术病例的次均住院费用水平的动态变化,采用卡方检验分析次均住院费用构成比变化。结果:2013─2016年,胆囊切除术病例的次均住院费用水平总体增长10%左右,其药占比显著降低,耗材费占比、医务性劳务费占比、检查费占比均显著升高。结论:我国医务人员劳务价值逐渐体现,药占比得到控制,而检查费占比和耗材费占比却显著升高,住院费用结构有待进一步优化。

Objective： To analyze the composition changes of hospitalization expenses from 2013 tp 2016 and provide suggestions for further reform of medical service price and payment policy in China. Methods： Using the sequential and base ratio to analyze the changes of hospitalization expanses and its composition ratio. Chi-square test was applied to analyze the composition of average hospitalization expense. Results： In 2013-2016, the hospitalization expanses of cholecystectomy increased by 10%. Among them, the proportion of drugs was significantly reduced, while the proportion of consumables, the proportion of medical services and the proportion of inspection were significantly increased. Conclusion： The labor value of medical personnel was being reflected in China, and the proportion of drugs was controlled. However, the proportion of medical consumables and examination expanses were increasing significantly. The structure of hospitalization expenses needed further optimization.
