甲状腺原发淋巴瘤18例临床病理特征分析

Clinicopathological Characteristics Analysis of 18 Cases of Primary Thyroid Lymphoma

目的通过分析18例甲状腺原发淋巴瘤的临床病理特点，总结其形态学及免疫表型特点，探讨其发病机制。方法回顾性分析18例经手术切除甲状腺淋巴瘤的临床病理资料，结合常规病理切片及免疫组化表型进行观察。结果甲状腺原发淋巴瘤好发于女性（16／18），临床常表现为甲状腺弥漫性肿大，其中黏膜相关淋巴瘤13例，弥漫性大B细胞型淋巴瘤4例，1例为黏膜相关淋巴瘤，部分区域伴有弥漫大B细胞淋巴瘤转化。在淋巴瘤的基础上同时伴发甲状腺乳头状癌的病例有2例。淋巴瘤伴有桥本氏甲状腺炎的病例有6例。所有病例具有高表达B细胞标志，低表达或不表达T细胞标志，所有病例均表达CD45。结论甲状腺原发淋巴瘤是一类少见的，具有特殊临床病理特征的恶性肿瘤。

Objective To explore the pathogenesis of primary thyroid lymphoma by analyzing its clinical and pathological characteristics, summarizing its morphology and immune phenotypic characteristics. Methods The clinical pathological data of primary thyroid lymphoma were studied in 18 cases, which were diagnosed by histopathology and immunohistochemical features. Results Primary lymphoma of the thyroid gland occurs in women. The clinical feature is the diffuse enlargement of the thyroid gland. There were 13 cases of mucosa associated lymphoma and 4 cases of diffuse large B cell lymphoma. And lymphoma and papillary thyroid carcinoma occurred in 2 cases, there were 6 cases of lymphoma with Hashimoto＇s thyroiditis. All cases had high expression of B cell markers, low expression or non expression of T cell markers, and all cases expressed CD45.Conclusion Primary lymphoma of the thyroid gland is a rare type of malignant tumor with special clinicopathological features.
