埃索美拉唑联合阿莫西林及克拉霉素治疗幽门螺杆菌阳性消化性溃疡的效果观察

Effects of esomeprazole combined with amoxicillin and clarithromycin in treatment of Hp positive peptic ulcer

目的观察埃索美拉唑联合阿莫西林及克拉霉素治疗幽门螺杆菌(Hp)阳性消化性溃疡的效果。方法选取该院2016年3月‐2017年6月治疗的Hp阳性消化性溃疡患者78例为研究对象,按照随机数表法将其分为对照组和观察组,对照组给予阿莫西林和克拉霉素治疗,观察组在对照组基础上联合埃索美拉唑治疗。比较两组患者治疗效果、血清因子水平、Hp根除率及用药不良反应发生情况。结果观察组患者治疗总有效率为94.87%(37/39),高于对照组患者的79.49%(31/39)(P<0.05);治疗后,对照组血清炎症因子白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平均高于观察组患者(P<0.05);治疗后,对照组患者Hp根除率为79.49%(31/39),低于观察组患者的97.44%(38/39)(P<0.05)。结论埃索美拉唑联合阿莫西林及克拉霉素治疗Hp阳性消化性溃疡患者临床效果显著,可有效降低患者溃疡炎症反应,提高Hp根除率,改善腹痛不适症状,加快患者康复进程,且用药安全可靠。

【Objective】To observe the effects of esomeprazole combined with amoxicillin and clarithromycin in treatment of helicobacter pylori（Hp） positive peptic ulcer.【Methods】Seventy-eight patients with Hp positive peptic ulcer treated in our hospital from March 2016 to June 2017 were selected as the research objects, and they were divided into two groups according to random number table, with 39 cases in each group. Control group was given amoxicillin and clarithromycin, while observation group was treated with esomeprazole based on control group. The curative effects, serum factors, Hp eradication rate and adverse reactions were compared between the two groups.【Results】The total effective rate of observation group [94.87%（37/39）] was higher than control group [79.49%（31/39）]（P〈0.05）; after treatment, the levels of interleukin-8（IL-8） and tumor necrosis factor-α（TNF-α） in control group were higher than those in observation group（P〈0.05）; after treatment, the Hp eradication rate of control group [79.49%（31/39）] was lower than that of observation group [97.44%（38/39）]（P〈0.05）.【Conclusion】Esomeprazole combined with amoxicillin and clarithromycin in treatment of Hp positive peptic ulcer is effective, it can decrease ulcer inflammatory reaction, increase the Hp eradication rate, improve abdominal uncomfortable symptoms and accelerate recovery, and it is safe and reliable.
