腹腔镜胆囊切除术联合优质干预对患者术后胃肠功能及睡眠的影响

Effect of Laparoscopic Cholecystectomy Combined with High Quality Intervention on Postoperative Gastrointestinal Function and Sleep in Patients

目的:探讨腹腔镜胆囊切除术联合优质干预对患者术后胃肠功能及睡眠的影响。方法:选取2017年1月至2017年10月解放军第102医院收治的行腹腔镜胆囊切除术患者50例,随机分为观察组和对照组,每组25例。对照组采取常规护理,观察组在对照组基础上实施优质护理干预,比较2组术后的胃肠功能恢复情况及疼痛评分;比较2组患者术后入睡时间、睡眠时间及PSQI评分。结果:观察组的肛门首次排气时间、肛门首次排便时间、肠鸣音恢复时间均明显短于对照组(P<0.05)。观察组术后24 h、48 h、72 h的VAS评分均显著低于对照组,差异有统计学意义(P<0.05)。与对照组比较,术后观察组患者入睡时间、睡眠时间和PSQI评分均显著降低(P<0.05或P<0.01)。结论:腹腔镜胆囊切除术联合优质干预可有效促进患者术后胃肠功能恢复,减轻疼痛,并显著提高患者术后睡眠治疗,效果显著,值得在临床大力推广。

Objective： To investigate the effect of laparoscopic cholecystectomy combined with high quality intervention on gastrointestinal function and sleep after operation. Methods： From January 2017 to October 2017,50 patients treated with laparoscopic cholecystectomy in 102 Hospital of PLA were randomly divided into observation group and control group with 25 cases in each group. Quality nursing intervention was performed on the basis of the control group to compare the recovery of gastrointestinal function and the score of pain after operation,and to compare the time of falling asleep,the time of sleep and the score of PSQI after operation in the two groups. Results： The time of first anal exhaust,the time of anal first defecation and the recovery time of bowel sound in the observation group were significantly shorter than those in the control group（ P〈0. 05）. The VAS scores of the observation group at 24 h and 48 h after operation were significantly lower than those in the control group（ P〈0. 05）,and the difference was statistically significant（ P〈0. 05）. The sleep time,sleep time and PSQI score in the observation group were significantly lower than those in the control group（ P〈0. 05 or P〈0. 01）. Conclusion： Laparoscopic cholecystectomy combined with high quality intervention can effectively promote the recovery of gastrointestinal function,relieve pain,and significantly improve the postoperative sleep treatment. It is worth popularizing in clinic.
