慢性乙肝患者焦虑抑郁心理对睡眠质量的影响研究

Study of the Effect of Anxiety And Depression on Sleep Quality in Chronic Hepatitis B Patients

目的:探讨分析慢性乙型肝炎患者的焦虑抑郁心理,及其对患者睡眠质量的影响。方法:慢性乙肝患者和健康人群均采用焦虑自评量表(SAS)和抑郁自评量表(SDS)进行焦虑抑郁心理状态进行评分;采用多导睡眠监测(PSG)评估客观睡眠质量,匹茨堡睡眠质量指数量表(PSQI)和睡眠自评量表(SRSS)评估主观睡眠质量。结果:慢性乙肝患者的SAS、SDS评分分别为(54.86±7.49)分和(57.32±8.51)分,均显著高于健康人群(35.97±5.17)分和(36.48±4.30)分,P<0.01;与健康人群比较,慢性乙肝患者的总睡眠时间(TST)、Ⅰ期睡眠(S1)、Ⅱ期睡眠(S2)、慢波睡眠(SWS)、快动眼睡眠相(REM)、睡眠效率均显著缩短,差异具有显著性,P<0.05;且睡眠潜伏期(SL)、长觉醒次数和短觉醒次数有明显的增长或增多,P<0.05;慢性乙肝患者的PSQI、SRSS评分分别为(14.32±1.51)分和(31.53±5.83)分,均显著高于健康人群(10.27±1.17)分和(24.41±4.04)分,P<0.05。结论:慢性乙肝患者较健康人群,存在明显的焦虑抑郁心理,且极大的降低了患者的睡眠质量。

Objective： To investigate the anxiety and depression of chronic hepatitis B patients and its effect on sleep quality. Methods： Chronic hepatitis B patients and healthy subjects were assessed by anxiety and self-rating scale（ SAS） and depression self-rating scale（ SDS）. The objective sleep quality was assessed by polysomnography（ PSG）Fortune Sleep Quality Index（ PSQI） and Sleep Self Evaluated Scale（ SRSS） to assess subjective sleep quality. Results： The scores of SAS and SDS in patients with chronic hepatitis B were（ 54. 86 ± 7. 49） and（ 57. 32 ± 8. 51）,respectively,which were significantly higher than those in healthy subjects（ 35. 97 ± 5. 17） and（ 36. 48 ± 4. 30）,P〈0. 01. Compared with healthy people,the total sleep time（ TST）,stage I sleep（ S1）,stage Ⅱ sleep（ S2）,slow wave sleep（ SWS）,fast eye sleep（ REM） and sleep efficiency were significantly shortened,and the differenceswere significant,P〈0. 05. And the number of sleep latencies（ SL）,long awakening times and short awakening were significantly increased or increased（ P〈0. 05）. The scores of PSQI and SRSS were（ 14. 32 ± 1. 51） and（ 31. 53 ± 5. 83）,respectively,which were significantly higher than those of healthy people（ 10. 27 ± 1. 17） and（ 24. 41 ± 4. 04）,P〈0. 05.Conclusion： There is obvious anxiety and depression in healthy patients with chronic hepatitis B,and the quality of sleep is greatly reduced.
