益肾祛痹汤治疗肝肾亏虚型膝骨关节炎的临床疗效评价及对关节液中炎性因子的影响

Clinical Evaluation of Yishen Qubi Decoction in Treating Knee Osteoarthritis Due to Deficiency of Liver and Kidney and Its Effect on Inflammatory Factors in Joint Fluid

目的:观察益肾祛痹汤治疗肝肾亏虚型膝骨关节炎的临床疗效。方法:选取96例膝骨关节炎患者按随机数字表法分为2组,对照组患者采用口服双醋瑞因胶囊联合塞来昔布胶囊治疗,观察组在对照组治疗的基础上采用益肾祛痹汤治疗,比较2组患者治疗前后关节液中的炎性因子水平、膝关节功能、中医证候积分、生存质量及临床疗效评价。结果:治疗后观察组患者关节液中炎性因子除TIMP-1变化不甚明显外,其余炎性因子均明显降低并显著低于对照组;治疗后观察组的Lequesne指数评分、WOMAC评分及VAS评分均显著低于对照组;治疗后2组中医证候积分比较,观察组除神疲乏力、畏寒肢冷外,其余各项评分显著较对照组低,观察组患者的临床总有效率明显高于对照组。结论:益肾祛痹汤治疗肝肾亏虚型膝骨关节炎的疗效确切,并可有效调节关节液中的炎性因子,为临床用药提供有益参考。

Objective： To observe the clinical effect of Yishen Qubi Decoction in the treatment of liver and kidney deficiency type knee osteoarthritis. Methods： 96 cases of knee osteoarthritis were randomly divided into 2 groups. The control group was treated with oral Diacerein Capsules combined with Celecoxib Capsules,and the observation group was treated by Yishen Qu Bi Decoction on the basis of the control group. The levels of inflammatory factors,knee joint function,TCM syndrome score,quality of life and clinical efficacy were compared between the 2 groups before and after treatment. Results： After treatment, the inflammatory factors in the articular fluid of the observation group were significantly lower than those in the control group,and the Lequesne index score,WOMAC score and VAS score of the observation group were significantly lower than those in the control group. After treatment,the 2 groups of TCM syndrome scores were compared. Except for fatigue and arersion to cold,the scores of the observatiion groups were significantly lower than those of the control group. The total effective rate of the observation group was significantly higher than that of the control group. Conclusion： Yishen Qubi decoction is effective in the treatment of knee osteoarthritis of deficiency of liver and kidney,and can effectively regulate the inflammatory factors in the joint fluid and provide useful reference for clinical medication.
