针灸治疗脑卒中后吞咽障碍用穴规律研究

Law of Acupoint Selection in Acupuncture Treatment on Dysphagia After Stroke

目的基于数据挖掘技术,分析针灸治疗脑卒中后吞咽障碍的用穴规律。方法收集1997-2017年中国期刊全文数据库(CNKI)中针灸治疗脑卒中后吞咽障碍的相关文献,严格依据纳入、排除标准进行筛选,建立处方数据库,从腧穴选用、腧穴配伍、部位归属、经络归属等方面对用穴规律进行分析。结果共纳入文献283篇,针灸处方390条,腧穴以廉泉、风池、金津、玉液、翳风使用频次最高,高频腧穴间的相互组合是常见配伍形式,腧穴分布以头面颈项部为主;所选腧穴经穴、非经穴占比分别为73.78%、26.22%,经穴遍布十四经,以足少阳胆经、任脉、督脉最为常用。结论针灸治疗本病以局部选穴为主,配合远端选穴;优先选用廉泉、风池、金津、玉液等高频次腧穴进行处方配伍;重视阳经腧穴的运用,并辅以针对性强的非经穴。

Objective： To analyze the law of acupoint selection in acupuncture treatment on dysphagia after stroke based on data mining technique. Methods： A prescription database was estabalished by collecting the standard literature of acupuncture treatment on dysphagia after stroke from China journal full-text database（CNKI） during 1997-2017,and then analyzed its law from the point of acupoint selection,acupoint compatibility,location attribution,meridian attribution and other aspects. Results： 283 literatures and 390 prescriptions were included. Lianquan,Fengchi,Jinjin,Yuye,Yifeng were most frequently used and the combination of these high frequency acupoints was a common form of compatibility. The acupoints were mostly distributed in the neck and head. The acupoints of the selected acupoints were 73.78% and 26.22% respectively,and the acupoints were all over the 14 meridians,with the gallbladder meridian,ren meridian and the governor meridian most commonly used. Conclusion： The selection of acupoints is mainly on the local affected area,combined with distal selection.High frequency acupoints are preferred for prescription compatibility. It emphasizes the use of the acupoints of the Yang meridians,supplemented by the well-targeted non-meridian.
